AN NHS trust has apologised after a mental health patient killed himself during a canoeing trip - just a month after being rescued from the same lake following an apparent suicide attempt.
Mohammed Mansoor Elahi, 31, drowned at Hollingworth Lake, Littleborough, in 2013.
Yesterday an inquest jury ruled that staff at Birch Hill hospital in Dearnley, Rochdale, where Mr Elahi was a patient, were at fault for allowing him to take part in the trip.
The hearing had previously heard Mr Elahi, who had schizophrenia, had been rescued from the lake about a month before his death after running into the water.
