Attempts to save her life after the blunder ended prematurely because a doctor wrongly thought she did not want to be resuscitated.

A grandmother died after NHS staff ignored SEVEN warnings that she was allergic to penicillin, an inquest heard.

Susan Hammond, 61, had reacted badly to the antibiotic on two previous occasions ? including one injection which triggered a heart attack.

When the widow was admitted to hospital with a suspected chest infection her allergy was clearly marked on:

notes from Accident and Emergency;
a letter from her nursing home;
ambulance notes;
her Emergency Admissions Unit (EAU) form;
her prescription chart;
her doctor?s notes;
and a bright red allergy band she was wearing.
But Dr Viv Gunasekera still prescribed penicillin-based antibiotic Augmentin, which was then administered by nurse Susan Elliott.
