Older people and the very sick will be visited within two hours by a “rapid response team” of health and care staff under new NHS plans to relieve the strain on overcrowded hospitals.

The teams will include nurses, physiotherapists, occupational therapists, social workers and social care staff working for NHS trusts and local councils in England.

The “urgent community response” teams will operate 365 days a year. They will help older people and those with complex care needs whose health has suddenly deteriorated – through a fall, infection or exacerbation of an illness – try to avoid ending up in hospital.
Patient and medical groups have welcomed the teams, which are a key element of the NHS’s plan to provide increasing amounts of care in people’s homes and care homes.

This has been on trial for about six months where I live, it is called the Frailty Team, it has proved successful so they said it would go live around the UK. I have been one of the trialists, it started when I had a number of falls two of them down the stairs, so my doctor recommended me to the trial, all I can say is they are brilliant, I get home visits from the Nurse Practitioner, Podiatrist, Diabetic nurse and if required a Doctor. Its now been agreed after discussions with my Daughter and myself that having been assessed, my Daughter because she works full time and because of her shifts she can no longer give me the care that I need, So I have been offered my own flat in the Extra Care Complex, which has 24/7 care available, and is like a little village with restaurant, lounges, library with events arranged like dancing and film show once a week, the complex has 60 flats with your own private entrance. I am looking forward to it. I will let you know if I get through the process next Monday.

John.

I wish, I will believe it when I experience it in UK south, couple of years back they expected me to get myself to Xray in sub 0 temperatures, running a temperature and with pneumonia. Fortunately I had someone to drive that time, but I was so ill to ask a patient to go out in those temperatures with pneumonia and hospital already full of patients. Certainly do need the above service. I think these things get going in the North of the country before the South and I am not sure they always make it south.

Good Luck @ukjohn so glad you are already getting good care. They probably didn't consider me frail enough.

6 years ago now but waited 7 hours for ambulance to attend after mother fell. She was in her 80s had dementia and stroke victim, even 2 hours is long time to wait for help when your old sick & frightened.

Will Rosie be allowed? I hope all goes well for you John, I'm very sorry to hear you've been having problems, but it's great to hear you are being so well looked after 🙂

Northerner...Yes I am allowed to take Rosie with me, provided I put her in her cage when the nurses visit :D, there's no way I would go without her now having been together for 15 years.🙂

This system is aimed at the 80s and over, as I am 81 I suppose that I am a lucky one, up to now my experience of this system is fantastic, I feel like a kid being spoilt with the care and attention.

Jodee.. As I mentioned above it covers the over 80s, and I am in Bristol, which is South West 😉🙂

Interesting definition of the North, Jodee. UKJohn lives in Bristol. Last time I looked that was well South. :D
