A new 10m four-year plan has today been announced which will change the way primary care is delivered across Wales.

Deputy Health Minister Vaughan Gething said the plan will see primary care services enhanced so the majority of care is planned and provided in the community, closer to people’s homes.

It is hoped that it will also reduce unnecessary hospital visits.

Mr Gething said he wants the plan to a tackle the “root causes” of ill health and allow people to look after themselves more which will prevent people being admitted to hospital unnecessarily.
