The PM admitted today it was ?not acceptable? for NHS chiefs to avoid responsibility for scandals such as Mid Staffordshire, where 1,200 patients died needlessly.

David Cameron said managers must face ?proper accountability? when people were found to have died of neglect.

He also attacked NHS trusts signing gagging contracts with former staff.

Mr Cameron?s remarks came after he was challenged to explain why nobody had been brought to justice for the mass tragedy at Mid Staffs hospitals between 2005 and 2009 due to poor care and mismanagement.
