Hi diagnosed a week ago with a reading of 91 so have been making some changes to diet. In the mid afternoon I feel weak and shaky and my heart beats fast. when This happens do I need sugar or carbs I’m a bit confused. Thanks in advance.

If you have made some significant changes in reducing carbohydrates then it is most likely you body adjusting to a lower blood glucose level especially if you are also taking some medication to help.
A very small amount of something sugary like a jelly baby but also a low carb snack like some nuts or cheese.
If it keeps happening then speak to your GP surgery.
What are you having for lunch.

You could be making changes too quickly for a smooth transition, but you don't need carbs - though a warm drink and a tiny amount of carbs might get rid of the feelings if it is caused by the sharp reduction in blood glucose levels putting your brain is in a panic.
Are you able to test your blood glucose levels?
What would be a typical menu these days?
 
I had 2 ryvita with small piece of cheese and a tomato for lunch. I’m on merformin but only 2 day and raising slowly. Yes I’ve really stopped dead carbs so a big shock to my body I expect. And very anxious.
 
It is good that you’ve started making changes and would encourage you to stick at it, but you might be going at it too hard too fast. Be careful about making too many changes too fast, aince as well as the shaky side effects that you’ve found, it can also cause damage to your eyes if you improve your blood sugars too fast.

What people find is a good idea is to make a food diary of what they were eating and drinking and estimate the amount of carbs they were having and then reduce it by one third for a couple of weeks, then by another third for two weeks etc until they get to where they need to be to get blood glucose to a normal level, for some people that might be 130g per day, others as low as 50-100g per day. They would use a regime of testing before and 2 hours after meals to determine what that amount is.

Ryvita is just 'dead' starch - I would be having a large salad - if I ate lunch, but for a daytime meal, lots of salad, with a fair bit of cheese and maybe a couple of eggs - there is far more nutrition in that, colour, texture, vitamins and minerals.
Just going by how I feel, all the advice given to me about what to eat has been wrong for at least 50 years.

Glad to hear that you've been able to make some positive changes. My body took about 3 months to adjust to things so you may experience a few wobbles here and there until things settle down.

Have you been advised to test you blood sugar levels at all? it may be helpful so that you can see how your meals are impacting you and what you need to do when you feel shaky according to your numbers. I would call that a good low carb lunch, 2 original Ryvita is just under 14g carb, and even with a huge sweet tomato you would still be under 20g.
 
I can never understand anyone deliberately having a small piece of cheese. It's protein. Why limit it?
