Newly diagnosed - Gliclazide

Hi all
Diagnosed last week after routine blood test 91 hba1c. Have been put straight on Gliclazide which I have been taking for 1 week today. I immediately started to correct my diet and be more active and over the last few days, I have had 3 BG readings at less than 4, and 2 after meals of under 5. I also attended my DERIK course yesterday where the diabetes nurse says she doesn't understand why I was put on Gliclazide straight away. Has anyone experienced this, should I be requesting a review of my medication this soon?

Unless there's another condition causing problems (Kidneys, mainly) Metformin is usually the first choice of anti-diabetic drug.

It may be you are not eating enough carbohydrates for the dose of gliclazide which is a medication which induces the pancreas to produce more insulin so it is a balancing act of meds and carbs to avoid going too low. It may just be a period of adjustment for your body but keeping a food diary of everything you eat and drink with an estimate of the amount of carbs to provide some evidence to your nurse.
Often people are started on metformin and diet and if that is not working then add other medications but it may depend on individual circumstances and the policy of a particular GP of what to prescribe when HbA1C is as high as yours.
Normally people are aiming at 4-7 before meals and no more than 8-8.5 2 hours after meals but that is usually a gradual thing as it is kinder on the body, eyes and nerves to reduce blood glucose slowly.

The NICE guidelines on Type 2 Diabetes used to say that a Sulfonylurea such as Gliclazide could be prescribed to newbies of a particular kind. I can't remember which, need to check NICE again.
Why were you having a 'routine' blood test ?
What does DERIK stand for, it could be a sawn-off version of DESMOND. An educational course a week after diagnosis is good going.

A sulfonylurea should be considered for first-line drug treatment in adults with type 2 diabetes, if metformin is contraindicated or not tolerated and if they are not in either of

  • have chronic heart failure or established atherosclerotic cardiovascular disease, when should offer an SGLT2 inhibitor with proven cardiovascular benefit
  • are at high risk of developing cardiovascular disease, when should consider an SGLT2 inhibitor with proven cardiovascular benefit
