I need a little advice.
I've been diabetic for the last 8years and have been managing it well until diabetic consultant changed my insulin from lantus to tresiba.
She told me it last for 2days and to only inject in the morning one day and the evening the next as can last upto 42hrs.
My readings have gone from a range of 6-9 , now at 15-20.
Do I increase units or inject every morning???
 
Welcome @LouG76 🙂 Why was your insulin changed? I’ve never used Tresiba but I think @Bloden and @mum2westiesGill use it. As far as I understood they take it once daily but I’m not totally sure so hopefully someone can confirm that for you. The guidance says daily too.

She said it was a better insulin for me to use, I'm sure it is once I get my readings back on track. Thank you.
 
Hi there @LouG76. In my experience, there’s usually a bit of a BG wobble when changing to a different basal insulin. Mind you, if your BG was 6-9 with Lantus, I’m wondering why your consultant has switched you to Tresiba.

I switched from Lantus to Tresiba too - because Lantus was running out of steam after about 16 hours and leaving me high (literally o_O) and dry for about 6 hours every day. I was also fed up with Lantus’s peak, which’d randomly appear three or six hours after injecting.

As for timing, I take Tresiba once a day (in the evening), EVERY day. Maybe you need to ask your consultant a few questions. Are you taking a much lower dose now? I was on 16 units of Lantus. I currently take 11 of Tresiba.

Gosh I sometimes wonder where they get these consultants!!

Firstly, why would she change something which is working well for you and secondly, yes Tresiba is longer acting, usually considered to be about 36hrs activity I believe, but still designed to be injected daily. The overlap just means that you don't need to take it exactly the same time every day, not that you have to inject it morning one day and evening the next and then presumably have a day off and start again. That just sounds like a recipe for getting mixed up which is clearly what has happened with the consultant!
Personally, I would ask/insist to go back to Lantus if it was working well for you and you were happy with it. Tresiba provides a pretty flat line profile of insulin release when taken daily but many (probably most people) don't need the same level of basal insulin day and night. I only need about a third of my basal insulin overnight so I take 20 units of Levemir in the morning but just 6 at night at the moment.

I would be annoyed that someone who was in a position to help and support me with my diabetes management had caused me so much BG upheaval as you are clearly having, with no obvious need for change.... If you were struggling with Lantus I could understand a change but not if you were managing well with it.

Have you got a follow-up conversation booked to get updates based on how things have been going?

Were you aware of problems you were having with the Lantus that Tresiba was hoped to be better for?

I hope you get things sorted out asap.
I changed from Levemir to Tresiba (around July 2019) because I was having problems with Levemir - my then DSN wanted to swap me to Tresiba because she said it would stop me having low readings and give me a far more stable background. I was on 14 units of Levemir in an evening and 13 units in a morning.

The change over (I've still got the emails)
"On Monday morning 22nd July the dose of Tresiba will be 20 units. This is a reduction of your total daily dose of Levemir.

So that's when I started on Tresiba - I'm currently on 20 units which I take around 7am ish EVERY morning. I definitely don't take it in the morning one day and evening the next day.

I'm not really sure why she changed it other than she thought it would suit me better.
Unfortunately so far after 1 week struggling to control now.
I was currently on 20 units of lantus and 3-4 units of apidra.
Now 20bunits of tresiba and needing more apidra to cope with higher readings of 12-20
I'm taking things into my own hands now and starting today with 20 units every day.
See how that goes.‍♀️

Thinking back I wished I'd of said no to tresiba but I didn't realise it would be a problem.
Iam now starting to take tresiba everyday with regular testing and see how I go.

Many people who have switched to Tresiba love the flatter profile when they take it once a day.
However, I think it is preferred by men ... or people who do not experience monthly hormone fluctuations as changes can take 3 to 4 days to take effect.
