New Ypsopump

I have finally got my new pump. I actually felt a bit sad turning off my old pump and putting it away, it's been with me for a long time

I now have a Ypsopump closed looping with Libre 3 sensors. It's going ok, weird having to leave it alone. It seems to be correcting my post meal spikes but overcorrecting causing low BG levels, it then shuts off insulin and then I go high again - basically yo-yoing! When I say high, it's not really high, just out of range - highest was 11.1. Time in range currently 87% Time above and below target both about 7% and I've had 5 hypos.

I've been told to leave it alone as much as possible but I've decided to start pre-empting the lows and eating a small snack to prevent the low and entering it as a hypo treatment in the app. I'm hoping that putting it in the app as a hypo treatment will still allow it to learn but will stop the hypos.

Early days but surprisingly ok so far. I haven't tested it with any high carb meals yet but I'll give it a few days before I do that!
 
I'm exhausted! Anyone else find the first few days of HCL like this? I've had 8 hypos in 2 days and I think I'm approaching another one. I have chronic fatigue and a few other issues so I was tired before this but now I'm exhausted. I just hope it learns to stop over correcting me soon. I just wish I could tell it to stop! I just want to take control. I guess I'm just not good at taking a step back!
 
Do you think some of your settings are a bit off @Natalie123 ? I couldn’t deal with that either. It uses CamAps, doesn’t it? @Saoirse uses that with the Dana i pump so might have some tips about how to deal with this.

Thanks The algorithm only uses TDD, weight and carb ratios. The last 2 days I have used less insulin than my TDD so that might be confusing it. My carb ratios were working well previously so it probably isn't that. I might see if I can raise the target BG value to see if that helps.

It's probably not that bad really, I'm just very aware of it and not being able to react to it in the usual way is really weird! It's not a million miles off so I'm hopeful we will get along in a few weeks time!

Time in target changed - 3 wks back 71% Last week 77% This week 89%.

Those are brilliant results! I'm really hoping it will stabilise as it learns. At the moment I feel like I'm watching it constantly and eating more carefully (less carbs) than usual to try to help it out a bit although I know it's probably counter productive.

Sorry - had to giggle when I read that! It would be entirely out of character for shedloads of T1s NOT to 'watch a new pump constantly' - cos I know very well that I have with this one - but discovered on the occasions that I haven't watched it like a hawk - it's mostly behaved like I hoped it would anyway without my excess vigilance. Bloody hell - that was a nice surprise! 🙂 🙂
 
I don't know what I'm hoping to gain from watching it so closely since I can't do anything to help it! Glad I'm not the only one - I assumed I wouldn't be alone to be honest, it's hard to let go!
 
Can you change your correction ratio Natalie? I think you should be able to do this in settings. Might be worth considering as it seems to be the correction sending you low (do you think?)

Different algorithms do seem to have different settings / adjustments that can be changed (or can’t!).

I got the feeling from my clinic that CamAPS was one of the most tweakable (certainly in terms of setting a more forceful low target).

In your shoes I think I’d tweak the overall target BG so that it’s higher, to try to ditch a few of those hypos until it has started to settle a bit. Is there an exercise/activity mode? Those can reduce the feistiness of the algorithm’s response to things too.

Thanks @Lauren I think you're probably right. I've been told not to change anything yet but I might ask. The only problem is that the algorithm seems to be struggling with the opposite problem when I have a more carby meal so I think I might need the higher correction rate for those times unless I can adjust my carb ratios to somehow give me more when I'm eating lots of carbs. If I eat low carb, 30g of carbs or less (I don't count carbs in protein or veg unless it's root veg, sweetcorn or tomatoes) then my BG goes from 4 ish to about 7 or 8 at most and goes back down. If I eat 40g or more it shoots to 12 - 16 and stays there for about 2 or 3 hours before finally coming down. I'm tempted to just stop eating carbs and stick to the low carb meals but I don't feel I should have to do this!

Thanks @everydayupsanddowns I might try putting my target up a little during the day, but ideally I'd like to get the best control possible eventually. There is an ease off mode (for exercise) but I've been told to avoid using it unless I actually am exercising because the algorithm doesn't learn whilst the ease off mode is being used.

Easily done and very tempting to do!! Well done on the time in range, that's great. Mine is 84% so far but I haven't pushed it much yet, I've been boringly sensible. It hasn't dealt with going out for dinner or pizza yet!

It might be worth getting in touch with your Ypsomed rep and asking them for advice. I was (briefly) on the Ypsomed a few weeks ago and also struggled with correction times after eating carby meals (anything over about 20g!) so I understand the temptation to eat low carb. Bless you. I hope it all works out - your time in range is fab and I've heard once the system gets used to you the Ypsomed is great, so hopefully all will be ok.
 
Another new issue (or issues)... I'm having cannula issues. I'm using the micro orbit 2.0 steel cannulas which are the closest to the Medtronic ones that I was using before.

I'm only managing to get them through the skin 1 in 3 attempts and I can't tell if they are in properly because they have a piece of rigid plastic in the way. I'm not using the inserter, I really hate those things.

The rate that the insulin is given is so fast that it's painful.

When I take my cannulas out after 2 days they are red and swollen. My last one blocked after less than 24 hours and when I took it out the site was bruised, reddish purple and lumpy. I think this is because the pump is trying to give me so much insulin in one go (about 9 units in less than 30s in this case). I've decided to start splitting my meals into small amounts to spread the insulin out.

I feel like the cannulas are much less flexible and I am limited to 2 very small areas either side of my belly button where they will go in comfortably. This isn't going to be good for me long term. I've tried the usual areas that I could put the Medtronic ones but the needle won't pierce the skin or, if it does, it's really painful especially if I move, sit down or bend over.

Sorry for the long rant, I just feel really disappointed and miserable. I've had no contact with the diabetes nurses since I got my pump despite the rep who did my training saying that they will call me after 2 days. I know they are busy and short staffed so I don't want to waste their time.

If anyone has any suggestions, I'd really appreciate them. I probably should add that I have a needle phobia / anxiety and changing the cannulas has really thrown me. I tried an inserter for a cannula once and immediately sick from the shock / anxiety!

That’s surprising. I assumed most pumps allowed that. I think splitting the bolus up like you suggested sounds best. Hopefully it will help a little.
Thanks, I hope so. I've read that you shouldn't "stack" insulin but I don't think I have much choice! It's a shame because the pump and the algorithm are really promising and my time in range is much better already and it's only been a week, average of 83%, today is 93% compared to my previous 70% before this pump.
