Caveat: I don't have any experience (that I know of) in mixed insulin, though I did (once upon a time) use reasonably fixed doses of insulins which I'd guess weren't far off. (And I'm T1, of course, which is different.)

So you're taking a mixed insulin once a day in the evening? In that case don't take anything before lunch. (Same applies if you've been told to take two doses a day, in the morning and evening.)

I presume in a few months they'll review how you're doing and adjust it as appropriate. Maybe it'll be fine not to take insulin specifically for lunch. Or maybe you want to eat larger lunches and so insulin before lunch might be appropriate.

Alas I think you have rapidly discovered some of the limitations of premixed insulins - because they are adminstered twice a day, breakfast and evening (to distribute the long-acting insulin properly) you don’t really have the option of a big lunch :(

At least this is the way it works in T1.

I guess it would depend on how the insulin is working in the context of your T2, and how fully it is supposed to be replacing your insulin needs (or whether it is intended to act as more of a partial support to your own insulin production).

Can you get hold of your nurse or GP to ask?

As others have said, with mixed insulin you don't really have the option of a snack tea, unless you make the snack something carby e.g. toast which can be the same carbohydrate as a main meal for 2 slices of toast. If you just had something low carb, the insulin will be working on the expectation that you're eating a normal meal still and your sugar levels could go too low in the evening or overnight.

Most people prefer to take non-mixed insulins so that they can eat what/when they like and inject and adjust doses whenever they are eating, so that it is more flexible
