The warranty is up on my current Medtronic 640G and I've been offered the choice of a Medtronic 780G or Tandem t-slim. I'm hoping someone see either of these and can give me an opinion. I'm at a loss as to which to choose!!! I've been very happy with Medtronic and the Tandem t-slim in an unknown to me. Hope someone has more info than the site which only obviously gives a positive slant.

@mosie-rosie DPD have just delivered my new pump this morning. It will be my 4th pump and 2nd T-slim. I decided to stay with the T-slim as i like the integration it has with Dexcom, Glooko, and my apple watch / phone. Also having had the t-slim for nearly 4 years i have developed my own "tweaks" in addition to control-IQ to help stay in range. I'm currently running at over 80% in range which my consultant finds hard to believe given my lack of a pancreas. I have found a set-up that works for me, doesn't mean it would work for you, especially if you are comfortable with a Medtronic.

Hi @mosie-rosie
I switched to 780 two years ago in order to loop. It took some getting used to and to trust the pump to do a lot of what I had done before. I was not familiar with Medtronic pumps, so there were some changes to get used to but the support line were brilliant, as was the training available.

I understand that in manual mode the pump behaves very much like the 640 with some improvements. The big extra is the possibility of looping along with the Medtronic sensors. I use the Guardian 4 sensors which require 1 finger prick for the week!!!! I was doubtful whether this would be true but it is. Just one calibrations.

I use the pump in Smartguard which is their closed loop system. I now would not want to go back from this as it is working very well for me, and I find that I have plenty of time when I can ignore my diabetes, and just let the pump and the sensor chat to each other and do corrections as necessary. My favourite is the amount of sleep that I get, the pump settling things down in the first hour or so and then waking up back on target.

I did chat to DSN about a comparison between 780 and t-Slim and at present I would stick with my current pump as there is not enough benefit to make me want to change.

I will flag @everydayupsanddowns and @Sally71 who are both familiar with the t-slim, who can give you the pros of their pump.

If you have any questions do come back to me.

Welcome to the forum @mosie-rosie

I switched from MM640G to Tandem tSlim (with self-funded Dexcom sensors) a few years back. It’s probably the best kit I’ve had so far and has given me the best results - but it’s not about challenges and there are things to adapt to (I found).

The hypo avoidance part of the algorithm on the Tandem isn’t quite so forthright as Smartguard was for me, so I have to set alarms for early warnings around 5mmol/L so I can check whether I need to take action before Control IQ would kick-in.

Dex is great, and very accurate for me, but only updates readings every 5 minutes - and sometimes when my BG is misbehaving that can feel like an eternity! An update every minute would feel far less ‘laggy’. I’m not sure what the G7 is like, but currently only Dex G6 works with the tSlim.

Good luck with your choice!

How did you all manage to get a pump? I've been trying for 14 years but keeps getting fobbed off... I'm finally on the list apparently but heard nothing for months...

Sorry to hear you’ve had a difficult time accessing an insulin pump @mscharlottek :(

Some clinics seem to be more switched on and proactive about securing insulin pumps for appropriate candidates than others.

Great to hear you are on the list now. Have you chased things up? Ask them if there is anything they need from you to proceed, and push for a timeline. If mention is made of some meeting or other than needs to happen first - ask for specifics. When is it happening? Who takes the decisions? What are their contact details? Then chase up when that is meant to have happened.

Some clinics take a bit of prodding unfortunately!
