Mounjaro Advice Please

Hi,
My sugars have generally been uncontrolled for years no matter what medication I have been on. I have an anxiety disorder and depression for which i've always been treated. But this means I am an emotional eater. I don't have to be hungry to eat. So simply suppressing my appetite has never helped. My sugars have generally been up in the mid 60's. I was on gliclacide and Jardiance for a long time which just seemed to make my appetite worse and never lowered my sugars. But they kept me on it. Then Mounjaro appeared and around summer last year i was put on 2.5. It did nothing. It was raised to 5 which did nothing. Then 7.5 with the same outcome. Then 10 and suddenly I felt the difference. Not only did it suppress my appetite but it had a big effect on my emotional eating. Hard to explain but it also seemed to click off the switch that told me to eat even if not hungry. I was 11st 2 and am 5ft 4, when i started the 10. 6 months later i'm in a healthy weight, although at the higher end of 9st 9. I've dropped two dress sizes. More importantly though my sugars have gone from 65 to prediabetes levels at 44. However, in the past 7 days my appetite has returned with a vengeance. No idea why. So i contacted the diabetes nurse and asked if it could be raised to 12.5 in order to ensure my sugars didn't rise yet again and i didn't pile the pounds on. (I'm 64 and weight loss is absolutely secondary to me but a great addition).

Edit: I've just though why are not they not removing the glicazide or jardiance or both first? Doesn't make any sense to me to not stop the things that do not help but to halve the things that don't. Shouldn't they remove those tablets first?

She called and was quite short with me. She said we don't give it for weight loss only and your sugars are good. I told her that my appetite had returned and along with a little weight gain already I was sure my sugars were also going to rise and it is showing on my own monitor that they are slowly rising. She wasn't interested and cut me off and not only refused to raise she said in another 8 weeks they were cutting it to a maintenance level of 5. This had no impact on my eating before and won't now. When looking online and at recommendations by the makers etc a lot in my situation have a maintenance level of 10. She was very impatient with me and I have to be honest that I was so happy when my sugars came down, that this has left me worried and anxious.

After that long winded yammer I guess my question is, is this what normally happens if you reach a weight and sugar level they are happy with. Is the dosage always cut in half and what do I do when it all starts to rise as with emotional eating it inevitably will. She didn't let me discuss anything with her over the phone and was very curt with me. It was never told to me that this would happen if things went well. I just don't see the point of having done any of it because without a doubt the moment i go down to 5 everything will go back up. I have no control over it on an emotional level due to the conditions I have. In fact they didn't know themselves what should happen after you get to a good level and had to have a little meeting to decide. Surely the makers give that information on what should happen.

Anyone who is in this situation have any advice for me please? I'd be grateful because am very concerned. Is this a normal maintenance? If 10 no longer works for me very well (and i also know it will all rise again because the last 2 days of the week when i'm due to take it again my eating always doubles as I guess the injection is wearing off). How will 5 keep me at the levels i am now?

Hi, thankyou for your post it's very helpful to me as i was put on it in December, currently on my 2nd month of 5mg which doesn't seem to be doing anything, blood test in 2 weeks and then depending on results i think they'll put it up to 7.5.
HBA1C currently at 68.....

Yeah it didn't help me at that dose and they kept increasing by 2.5 increments until i felt the difference re emotional eating or it not supressing appetite. For me that was 10mg. My sugars were around the same as yours when i started the 10 and when sugars retaken around 2 months ago they were down to pre diabetes at 45 I think. Not gonna stay that way though if they lower to 5mg in a month :(You will hit a dose that is right for you and you'll see how amazing mounjaro is re sugars and weight loss.

Yes, it’s given for your blood sugars so if your hba1c is good now and you’ve lost the excess weight then they’ll gradually reduce your medication until you get to the minimum medication required to maintain your blood sugars.

Forgot to add, just in case, that as my dose highered I was getting more and more dizziness, light headedness every day. Sometimes were I had to sit on the floor quickly. It is not listed as a side effect but many are reporting it as one online. They checked heart and everything with me just in case but nothing wrong so presume it is the mounjaro doing it. As its pro's outweigh the cons it is something i am prepared to live with. Just didn't want you to worry if it happens, it is probably side effect of the higher dose.

Sounds like the higher dose has suited you well, but isn’t without its own challenges if you think it might be a factor in the dizziness you have been experiencing.

It’s unfortunate that the nurse has been a little brusque with you, ideally you’d want to be able to have an open and productive conversation, and to agree a way forward, perhaps maintaining a slightly higher dose on Mounjaro, but curring back on some of the other meds to see if you scan find a happy mid-point where your weight stabilises but without the dizzy light-headed spells?

I’m not sure if @goodybags has had Mounjaro reduced to a maintenance dose?

@suzanna valentine sorry to hear your DSN hasn’t shown you the understand your looking for at the moment :care: I’m not actually at my maintenance dose of Mounjaro yet @Leadinglights

For me the GP has now upped my Mounjaro to the 10mg dose, as he is hopeful I can actually further reduce my insulin my GP has said a few times I might need to actually go up to 15mg, currently the 10mg dose I’m finding actually quite harsh now I’ve lost a fair bit of weight since I started taking it,

in my head I’m kind of thinking im not far away from going to to a maintenance dose, I kind of feel will be less than the 10mg dose, but we will see.

Has your GP / DSN referred you a Dietitian yet ? @suzanna valentine:?
It might be worth speaking to one as obviously you’ve got your Hba1c down and want to keep it down plus control your appetite.

I’ve been on Mounjaro now for 8 months, previously other medication both Injectables and Oral Medication that for me didn’t agree for various reasons, sorry I can’t be of more help than tell you how it’s been for me, there are quite a few others on the forum that I know are on Mounjaro and I’m sure others who might not have posted the fact they are, maybe someone else can share their experience :thankyou: 🙄

I’ve just started my first week at 5mg, no negative side effects as yet so pleased with that, not expecting significant weight loss yet but I am definitely eating less, also I’ve cut my fast acting insulin by two thirds and slow acting by half, TIR sitting at 99%! There’s been no mention of cuts to other meds.

I am still taking it and the jardiance. They haven't mentioned trying to come off either of those.
