Hi have noticed some mornings I spike to 14, I take 5units off rapid insulin for 26 carbs which hospital advised, then find mibi an hour later going low. Anyone any tips as don’t want to be low at work

So when do you spike to 14? How long in advance of breakfast are you injecting your bolus?

This morning spike half an hour after eating, 10 minutes before eating

Then try moving your injection time earlier in 5 min increments. You should find that this reduces the spike. Go slowly and carefully, keeping your new time for two or three days so you can see how it works and if you need to change it again. Sometimes this also allows you to reduce your insulin dose (because you’re timing it better) and thus avoid dropping too low later @Woodywoodpecker

Thank you will try this tomorrow

Tried 15 minutes last few morning, Wednesday went to 14.2 but didn’t have great nights sleep, have noticed on those nights glucose usually higher in morning, don’t know if it’s just a coincidence. Thursday 11 but down quickly, this morning 10.4 then back in green. Thank you for your help

Another thought @Woodywoodpecker do you have the same breakfast each day? I can eat the same number of carbs on two days but from very different breakfasts. I find that a slice of OH’s whole meal bread doesn’t spike but if I am away and have cereal it is a lot more spikey.

You might find you still want to creep your pre bolus time further ahead of breakfast. It certainly looks like your timing of the bolus has already improved things. Just change things carefully in small steps, and as @Inka says leave it out for a few days before making another change.

A lot of us find it very helpful to focus on making changes to one meal bolus at a time, and starting with breakfast is often easier as it can be easier to get consistency with that meal.

Yes have the same every day, but thinking off switching it up, to be honest not a big breakfast person. No spike this morning, have done 3 days at 15 minutes. Was thinking tomorrow 20 minutes, and reducing one unit as busy day but not sure if a should reduce

It’s your insulin so you do whatever you think is best. It’s one injection not a lifetime commitment to the dose you choose, if you do 1u less and don’t like the results just go back to normal dose the day after.

Monday to Thursday cereal then work 8-1 will have fruit at 9 then 11 another snack biscuit, I weld protective clothing heavy work. Lunch at 1.20 sandwich yogurt, house work and mibi another snack, tea 5-5.30 just a normal tea tatties meat veg, Spag Bol etc, snack about 8pm. Weekend varies shopping housework walking, but meals about the same

Out of curiosity, do you inject insulin for each of those snacks or just rely on your previous bolus or physical activity or perhaps your basal insulin to mop them up? Not saying that there is anything wrong with snacking like that but just wonder how you manage it? I certainly have days when I graze rather than have full meals but I generally need to inject for each snack, unless they are very low carb, like a chunk of cheese, but a piece of fruit or a biscuit and cup of tea with milk would need a bolus injection for me.... I do like plenty of milk in my tea though! 😳

No don’t inject as under 20 carbs, as advised by hospital. I have been told to snack as need to put weight on, physical activity keeps it down as well

Tried 20 minutes this morning 4 units, spike to 11.7. I have changed cereal this morning 5 carbs more. Back to 5 units tomorrow, mibi try 4 for work mornings

Sounds like you are getting used to the inevitable juggling we do.
I definitely needed different basal/Bolus for work days as opposed to weekends, and also adapted according to what I had on each day.
Well done.
