Can anyone tell me why after changing my sensor & the warm up has finished am I always high in my glucose reading? Any advice would be appreciated.
 
Have you cross-checked with a fingerprick @kenyogi ?

Some sensors can take a little while to settle in, so it may be that the initial 24 hours might be giving you less reliable data?

I only used up to the G3 with Medtronic sensors, but don’t remember this being a regular occurrence with them.

Are your sensors part of a hybrid closed loop with the MM780? How long is the warm-up time on the G4s? Is there the possibility that it might just be a drift while your loop is not adjusting things for you?
 
Good morning @kenyogi I have found that the Guardian4 sensors give Reliable readings and, whereas with Libre I needed a settling in time before activating it, I no longer need this.

Whilst you are in warm up mode, your pump will be operating in manual mode. During this time your basal insulin is following the profile that you have active. Check that you have those set up correctly and the right one selected. (I had this problem when I have not selected one of my profiles I had set up and it was using one with no basal insulin being delivered)

Thanks for the replies. I use the Medtronic 780G with G4 sensors. The warmup is usually 2hrs. I only do a finger prick when requested to do so & usually they both correspond as near as. The manual basal doesn't kick in until the 4hr window for changing & warmup are completed. If the process takes longer then it goes to manual basal amount. Unless I am understanding something wrong??!!

I am using the same system as you. I am assuming that you are using it in Smartguard (looping) with the automated corrections going on as necessary every 5 min (whilst we can just let it get on with things - bliss) Apologies if that is an incorrect assumption. If you are using the sensors in manual mode you will need to go through doing a basal rate check, as well as checking carb and sensitivity ratios.

Whenever the system is not in Smartguard, such as during the 2 hr warm up or if it goes in to ‘sensor updating‘ for some reason, the basal rate is whatever profile you have set for when you are in manual. I have five of these set as that is what I did before I started looping. (When I got a new pump I forgot to set them all up and the pump was set to default on one that had a daily dose of 0 units!. Oops.)

Once you have done the finger prick, after warm up it will then switch to Smartguard and do the automated basal adjustments based on prior data and your sensor readings. If you are going high each time after/during the warm up period it would be worth checking:
- Basal rate profile settings
- Are you doing the change just after a meal? If so is your carb ratio/carb count correct?

When I get flipped out of Smartguard, it just reminds me that I still need my manual settings to be correct and I see just how effective Smartguard is at making adjustments where I have got things a bit out, or where the meal is particularly fatty, and previously I would have had to think about alternative bolus deliveries. Now I just let Smartguard sort that out for me.

Come back to me if any of this doesn’t make sense.

It has been running since the early days of the looping being available and you can see some of the issues that we found at the start. Do add anything in there. We are always glad to have other people’s tips.

Thanks for your reply. Yes all my settings are good & I do have Smartguard working, this is why I can't understand why my reads seem to be a lot higher after warmup has finished. Also thanks for the thread info.

Are you eating breakfast during the warm up? I wonder whether the rise shows just how helpful Smartguard is at dealing with wobbles after meals. Whilst you are in warm up these adjustments are not going to be working, so it is just the bolus that is working.

Do you pre-bolus for your meals? Even when using Smartguard I find this helpful in reducing the spikes after meals. My timings are different through the day and I know that on warmup morning I do not always manage to pre-bolus as effectively.

After that I can only think that it is your manual settings that need reviewing. I did find mine needed adjusting soon after I had started looping. It drove me bonkers to start with but then set aside a period of time to check these (whilst I was getting very stroppy with my pump as Smartguard just didn’t seem to work for me). Now very committed to looping and using tweaks.
