Stupidly I'm probably going to end up hypo at teatime because when I did my lunchtime carb count I realised I only did it for 1 round of bread and not 2 which is what I had
1 round of bread 18g
1x 4 finger kit kat 26g = 44g but should've been + an extra 18g so 62g


at 13:54 I had 3.5 units of humalog for 44g of carbs

then at 14:12pm when I realised I should've injected for an extra 18g of carbs I injected the extra 2.0 units

That’s only a short time after @mum2westiesGill 🙂 You should be fine. Just keep an eye on things but I wouldn’t worry. You noticed your mistake and corrected it quickly.
Thank you! It was 18 minutes. How often would you keep an eye on things? Don't forget I do finger pricking
 
I would do your usual blood tests but just remember that part of your lunchtime bolus was a little late. Keep that in your mind but it probably won’t make any difference 🙂

Obviously, if you feel ‘off’ in any way, do a blood test, but i think you’ll be fine 🙂

My usual blood tests for me is my teatime one.

I'll take that from you that I will be fine 🙂 thinking about it it's probably the same as injecting for a pudding if you choose to do it as a separate injection after your meal
 
If I understand your opening comments you say you injected for 1 slice of bread when in fact you ATE 2 slices so in fact you injected less insulin at that time so if anything you would be heading for Hyper [high] but you corrected for the 2nd slice which should bring you back to mid target range correctly.
You do need to be more careful and check, then check again before you act.
