Survey on experience discussing nutrition with healthcare professionals before during and after pregnancy

Hello! I am a researcher from the University of Southampton. We are currently surveying women living in the UK (ages 18-45).
We have developed a checklist that can be used in clinics as part of routine antenatal consultations, and the survey is to check if that would be acceptable for women living in the UK. In particular, we would like to hear from women who are living with Diabetes (Type 1/Type 2) or diagnosed with gestational diabetes about their experiences before/ during or after pregnancy.

Click here for the survey:https://cmj.surveysparrow.com/s/Nutrition-checklist-study/tt-a88928

We really value your time, this takes about 10 mins on average. You will be offered a chance to be included in a prize draw to win a £20 Amazon voucher. This is an anonymous survey and your name/ contact details will not be requested and leaving your email address to participate in the prize draw is optional.
Our study Facebook page can be followed to hear about updates: https://www.facebook.com/Nutrition-Checklist-study-103949311760781/
