Levemir Split??

Hi All

I was recently advised in Oct at my dafne course to split my levemir from once at night to twice morning and night.
so now i have 10units when i wake and 24 before i go to bed.

my question is would it hurt to take my night dose earlier say teatime??

today for example my bloods were 6.9 morning, 5.4 lunch, 8.3 dinner and 9.9 at 9pm so i was wondering whether the rise is due to the levemir trailing off and if i did it at tea time would it hold me steady in the evening?? my only concern would be rising over night if i did bring it forward??

any thoughts would be appreciated :D
 
If you are having the doses only 8-10 hours apart then it means your waking injection needs to last for 14-16 hours, so I would say it's quite possible that it is trailing off by 8-9pm. In that case, I wouldn't think there would be any harm in moving the evening injection so it is closer to 12 hours after the morning one. But I've never split basals, and I use lantus, so hopefully someone with split levemir can give you the benefit of their experience! Or give your DSN a call 🙂

Can I ask why you started to think/be told to split your dose, where your levels getting worse? I wondering wether to up my levemir, but maybe it is a not lasting the 24hrs thing and needs a double does/split instead?

Hi. I'm on a split Levemir dose. 6 units in a morning and 14 at night. When I did DAFNE I was told to inject background insulin as soon as I get up and just before going to bed. Whether or not this was twelve hours apart. This didn't work for me and I now inject on waking and then about 12 hours later which seems to work much better. I guess there's no science to it, you just need to do what works for you.
 
Emzi - There are 2 issues to consider in planning long acting insulin times: insulin profile and convenience / ease of remembering injections.

I use a different long acting insulin to you (Humalin I), so the profiles won't match, although even the same insulin can have different profiles in different people.
Personally, I find it easiest to have 1 injection on waking up and the other at bedtime time, each within an hour either way. Partly it's because I'm usually at home at those times and wearing nothing or a nightshirt, so easy to get to my thighs. However, no real problem injecting in other strange places eg hills and lakes when marshalling multi day & night adventure races. At tea time I could be anywhere.

So, basically, find what what works for you. With Levemir, it's best to test for 3 days before considering a change, so that you can spot patterns.

Id never heard of it til i went on the dafne course coz id always just had 1 injection of long acting at night but it was my DSN who advised it to try and lower my levels out. i find that with the split i tend to be more level and dont have as much in one go just spread out through the day, my problem is finding the right injection time for me and also matching my short acting ratios to my long 🙄

I am currently on a split and take my doses at 6:30am and 5:30pm, both with my breakfast and dinner, makes it easier to remember and gives me full coverage....................I have always associated Levemir with a split dose and Lantus being once a day, but I had never heard of them until I was told about DAFNE........
 
I was told by my DSN to spilt my dose of levemir, I do the morning one when I wake up and my night one at 10pm and it is not always a 12 hr difference between shots, but it works better than just one shot at night.
 
I split my lantus as I don't find it lasts and do it at 7-8am and 8-9pm. The other things to remember is that Levemir is quite dose dependant (dependent? I'm not sure) so the bigger the dose the longer it lasts. Your morning dose is smaller so may not be lasting as long. Do you use DAFNE online? maybe you could send a message to your educators through that for advice?
 
Hi i split my levemir 50/50 12 hours apart and everything works fine for me 🙂

Saying that all of us are different so maybe its best to discuss it with your DSN to see what they think if your not too sure 🙂
