Lantus - decrease

The last time I decreased was Friday from 18u to 16u BI and since then:

(jelly babies have gone and correction doses are back on the menu!)

When on Lantus I found 1u made a disproportionately massive difference in my levels.
 
Hi Gill,
Do you think this could be related to your monthly cycle at all? I've been playing around with different basals at different times of the month and at the moment I need 20-25% more basal insulin for the week or two before my period compared to the two weeks during/after. It took some kind person on here to point this out to me a year or so ago and then I looked and realised I was having two weeks of hypos followed by two weeks of corrections!
Might not be the case for you, but just a thought!

Maybe it's time this evening to increase by 1u.

I've been diabetic for 20 yrs and just can't get used to dialing up in 1u lol.
The pens I've got are:
HumaPen Luxura - for Humalog QA
ClikStar reuseable - for Lantus BI
which both go up in 1u!

I would give the bedtime insulin more time to see how it's go. Is he coming down with illness?

Have you done any forms of fasting tests??

I think you need to do these as it could be your carb to rapid acting that's out?

But Id get the basal levels sorted first and then look at the rapid acting.

Im going through it myself at the moment, trying to figure out whats happening we could do it together if you liked and post our results on here and compare?

Has anyone talked you through fasting tests? Sorry if im teaching a grandmother to suck eggs here!!

Hi,

I've done night time basal tests ie at 3am but never any forms of fasting tests so nobody has talked me through them. Tbh the only form of fasting test I've done is the one where I go for my 6 month HbA1c.

Not that I want to mention your age - but...are you nearing the Menopause? I only ask as the hormone changes must play a part in affecting levels in the same way they do for a cycle. I know many ladies who have monthly changes in levels but if this isnt a problem for you because of age then perhaps it is the menopause.🙂Bev

If I were you I would ask for tests at the GP's to see whether it is or not. If you find out it is then it would give you some answers I think as I know what hormone changes do to Alex - his needs change weekly at the moment! Perhaps it is the same for you? It might answer the question of why you seem to spend a few weeks on low levels and then suddenly up in the high teens. I would also ask for the CGM as it might help to work out a pattern for hormones.😉🙂Bev

p.s. I meant that Alex's teenage hormones play havoc - not his Menopause - he he!
 
The basal test is only something ive recently discovered, Ive been on lantus for about 8 years, but it sounds stupid but ive never really known its purpose until recently through forums etc. The idea of your lantus (basal) is too keep your levels as flat as possible throughout the day with the odd peak and trough. So in an ideal world if you started the day at 6mmol, and you did not eat, and did not do any fast acting, corrections etc etc, your blood glucose would remain around 1.7mmol either side of this starting number throughout the whole 24 hours. The idea then is that the fast acting counteracts our meals and when the food has been digested and the insulin has been spent we remain in our flat state for longer.

With our MDI regimes we can only strive to get as close to this perfect world as possible. By doing fasting tests we can see how stable our bloods are when not eating and not injecting fast acting.

In my quest to gain control of my diabetes, this is my first step... is my basal level correct? Once I know it is I can work on my fast acting ratios.. .without knowing if your lantus is right its hard to know what outside influences are playing with your fast acting ratio.

take a look at that link it walks you through basal testing. Split the day into four and try that. Always eat a healthy meal at the meal previous to starting the test, not much fat etc..!

Errr, it's over several days, not all on one day. You do a time block then forget about it till the day after next, do the next one etc and do em in any order you like to fit in with your life.

Instead of completely missing out lunch or whatever, have it late or the previous meal early. As long as the fast-acting from the previous meal has run its course that's enough to start the 'fast'.
 
oopsy..! typo kid of. yes 4 different periods over differnt days. i couldnt possibly go longer than 8 hours without eating!!
