The Health Secretary said he had given officials until next April to draw up and implement a plan aimed at helping those with chronic conditions who are the ?heaviest users of the NHS?.
He told the NHS Confederation conference in Liverpool that the plan would relieve the ?huge pressures? experienced by A&E departments, which reflect problems in the wider health service.
Tackling the underlying problems behind the crisis in A&E, including high demand from older patients, is essential if the burden is to be addressed, he said.
The plan will involve boosting public confidence in community-based health services, improving coordination between health and social care services and making GPs responsible for patients? well-being after they leave hospital.
