Apologies for the photo. Someone sent it to me this morning. He described himself as insulin dependent. He said he has been told three times that it’s nothing to worry about, but I’m not convinced. I’d welcome other’s thoughts.

Told by whom - a doctor?
He needs an appointment with a dermatologist asap.

Nevertheless he is obviously concerned and so are you. If the hospital and the locum GP told him not to be concerned then they should also have told him what it was, how to avoid it and what to do to treat it. Did they do any of those things? Did they tell him the name of the condition? I have done a quick search of the 100 most common skin disorders but to no avail. This is hardly surprising given that I have only one photograph to see. To dismiss it as something 'not to be concerned about' is, only in my opinion of course, totally irresponsible and not conducive to the well-being of the patient.
 
Does it feel hot to touch? Is it itchy/any loss of sensation etc? Also hard to tell from picture but is it swollen in comparison to other one?
Even if its not diabetes related so not a worry from that perspective if its causing discomfort I would encourage a second opinion or further referral to dermatologist if the hospital opinion was from the diabetes specialist. Do you know if they've been offered treatment?
 
Might be nothing much. However, they need to get answeres from the HCP as to what it is.

It looks like they have some sort of infection going on.
I've had several infections in my legs (cellulitis) with the above symptoms. I don't recall the greyish skin. I also felt absolutely dreadful before it showed. How do they feel?
How fast has this happened? For cellulitis, it comes up quite quickly from what I'd know.
 
Something wrong there @Bronco Billy, as @LucyDUK says does it feel hot?
I have problems with my feet and they are checked every six weeks especially
for Charcot foot and ulcers. Looks very similar to me and as a D HCP should be
taking much care of lower legs and feet problems, you are right to be concerned.
 
Hi. I would look for a second opinion. Is your friend a T2 with excess weight? How well-controlled is the blood sugar?

He looks it from photos I've seen. He's a facebook 'friend' and I've never met him in person
 
Get him to call NHS111 if his gp isn’t being useful. Or go to a pharmacist as a first port of call perhaps.
 
Blimey, the differential diagnoses would run to half a page. Questions: Does he work? if so where? Where does he live? Does he have a pet dog that trots around through the grass? Does he go for walks in the country? In any event, it obviously needs antibiotics.In my view, a trip to A&E would be more appropriate.
 
..... and what pray is the thing that looks like a very angry zit smack in the middle of all that, or the rash?
 
@trophywench I thought that was the thing he was worried about!
 
Whatever it is there is a line of demarcation of infection/inflammation around it.
 
I wondered if it were a tick or if eg a grass seed had got itself embedded? Lots of folk have been gardening and/or going for walks 'just lately'.
 
Could be a bite that’s progressed to cellulitis. Strangely however, his ankle has the same scaly rash. I’d want a definitive diagnosis especially if it’s hot and the redness is spreading. It’s an added risk with diabetes.

I am surprised doctors don't think it is anything to be concerned about. Has he been bitten do you know? It could be an infected bite.
