Good evening. I’ve asked lots of questions on this forums. Always get good advice.
I have recently had my Humulin I dose split from 79 units once per day to an increasing dose twice per day , depends on my readings. I was advised to keep increasing the doses into I get into single figures. My local specialist Diabetic nurse said to have a Hba1c in 5 weeks then I will be discharged from the service.
My questions are
- is there a maximum amount? Do I just keep going
- should I not be offered a alternative insulin? Surely it’s not one fits all ?
Thank you

Basically you are being treated as a 2nd class citizen with the way you are being treated :(

My personal view is that expecting someone to use just a basal insulin to cover both basal and bolus is dangerous.
What happens if you become ill ie., vomiting thus can not eat? You will have all this background insulin swimming around in you with no carbs to soak it up.

Insist you are put on a basal/bolus routine so you can control your levels and the food you eat.

Thank you Pumper-Sue . Tbh I was slightly disappointed. It appeared that I was getting rushed to be discharged .

It may be worth discussing alternative insulin therapy options @CELTICDUFF

Background-only therapy can help if your pancreas still has quite a bit of insulin production, but can’t quite keep up with requirements. if your insulin production is struggling a little more, then the addition of a mealtime (rapid) insulin may give you a more flexibke system. That way you can tailor your humulin I only to cover your background needs, and have a separate mealtime insulin to help with mealtime carbs?

Strangely the have changed. Previously my morning reading was always the highest , they are now around 10-12 mmols
Evening is 19- 29 mmols .

It looks like your evening dose copes better when you are not eating but is still a bit high but in no way does your morning dose cope with the carbs in your meals during the day and your evening readings are WAY too high. At those levels I hope you have a means of checking for ketones particularly if you feel unwell.
As mentioned a different regime would be better.

To answer the question, no there isn’t a maximum dose. It’s normal to only start type 2s on one dose of long acting insulin and not start a short acting immediately. If you can’t manage your bg levels with just the long acting though then they should add a fast acting, you can ask about that too

I dose is long acting insulin 104 units and short acting insulin 7 units at dinner time.I was given the same advice as you were to increase long acting insulin until my sugar levels were around 8.I was also discharged back to the GP and they got no one any good to deal with diabetics.I didn’t feel ready for this as I not confident using short acting insulin. But they have to many patients and not resources.
