Hi
I was prediabetic for a few years
Then last november I hit 48mmol (so diabetic)
I was not told by my gp that I was now diabetic.
I, after seeing my hba1c test result, had to phone them and ask......

No appology, just told yes I was
Sent for foot and eye screening....

3 months later (feb 24), after a real change to diet etc I tested at 45mmol/mol...
Good news....

Contacted gp in June to ask why I had not had a further hba1c test (nhs guidance)
and was told I didnt need it as it was well controlled.....

Am I correct that....
1. 1st year after diagnosis nhs recommend 4 hba1c tests
2. After 12 months tests should be every 6 months
3. 1 test after diagnosis is not enough to porrve stability

Or
Is the gp right to act as if I am back to being pre- diabetic

Btw
Just had second appoint ment for eye screenib service (so they seem to be doing it right)

You will probably find that different surgeries will implement to guideline differently'
But it is usual to have a HbA1C test after 3 months to check id measures being taken are beig effective and then if things are going in the right direction after another 6 months and thereafter if it is going the right direction after another year and then annually as well as the foot and eye checks and maybe some other blood tests, kidney and liver function, Folate, Vit D, B12, cholesterol, full blood count.

1.6 Blood glucose management

HbA1c measurement and targets

Measurement

1.6.1

Measure HbA1c levels in adults with type 2 diabetes every:
  • 3 to 6 months (tailored to individual needs) until HbA1c is stable on unchanging therapy
  • 6 months once the HbA1c level and blood glucose lowering therapy are stable. [2015]

1.6.2

Measure HbA1c using methods calibrated according to International Federation of Clinical Chemistry (IFCC) standardisation. [2015]

1.6.3

If HbA1c monitoring is invalid because of disturbed erythrocyte turnover or abnormal haemoglobin type, estimate trends in blood glucose control using one of the following:

  • quality-controlled plasma glucose profiles
  • total glycated haemoglobin estimation (if abnormal haemoglobins)
  • fructosamine estimation. [2015]

1.6.4

Investigate unexplained discrepancies between HbA1c and other glucose measurements. Seek advice from a team with specialist expertise in diabetes or clinical biochemistry. [2015]
