The NHS in Surrey failed to prevent or may have even caused the death of a 26-year-old Oxford graduate, an inquest jury has concluded.
Sarah Shepherd, from Godalming, died at the Royal Surrey County Hospital in Guildford on September 13 2011, after being found the previous day in her room in the Noel Lavin Unit at Farnham Road Hospital.
Access to the room had been blocked from the inside by her wardrobe and she had a plastic bag over her head.
The inquest into her death began at Woking?s civic offices on Monday November 4 and a narrative verdict was given last Friday (November 15).
