I just realized that I am at risk of Diabetes, Anyone knows how to change my lifestyle smoothly?? I am really stressed after hearing that news...

Many in your situation who make some modest changed and move themselves out of the at risk category.
Depending on where you are in the range HbA1C 42-47mmol/mol of 'at risk' will indicate how much change you will need to make.
Many feel they have a healthy diet but sadly if your body has a problem with carbohydrates not just sugar then those 'healthy' carbs are not so good. Cutting down on portions of rice, bread, pasta, pulses, tropical fruits and juices as well as things like cakes, biscuits and sugary drinks.
I assume from you name you are from Asian background (I apologise if you are not) so it can be tricky but meat, fish, eggs, cheese, vegetables and salads and fruits like berries are all good.
You may find the book Carbs and Cals World Foods useful for carb values of a whole range of foods to give you an idea of portion sizes and carbs.
Many who adopt a low carb regime when diagnosed as diabetic (over 47mmol/mol ) will aim at no more than 130g per day carbs but if you are at the lower end of the range may be able to a bit more lenient on that.

Your aim is to reduce your HbA1C to normal below 42mmol/mol but just as important not to ignore your situation and risk going over than threshold.
