Nurse practitioner Eileen Sosa worried about patient Felicita Torres’ roller-coaster blood glucose readings.
A 15-minute office visit wasn’t long enough for Sosa to figure out what was causing Torres’ elevated blood sugar.
The answer came only after community health worker Madeline Diaz began paying weekly visits to Torres’ southeast Lancaster city home.
Torres, a 72-year-old diabetic, revealed that she didn’t always have transportation to the pharmacy, so she tried to conserve insulin by taking only half of her daily dose.
Diaz helped Torres regulate her medication and diet. Six months later, Torres’ blood sugar is under control.
