A newbie I am to this site.A question if I may... Does anyone have any experience of coming off of Toujeo and going back on to Lantas?Is it a case of waiting 36 hours from your last Toujeo injection to your first Lantas injection.I live in Mallorca and the wait to see the diabetes nurse is far too long.Toujeo doesn't seem to work so well for me.Any help would be greatly appreciated.

I am sorry but I have no experience of using Touejo.
Is there a reason why you are switching back.

When on MDI I switched from Lantus to Levemir so that I could split my background insulin dose. This enabled me to make adjustments to the evening dose and reduce night hypos, without adjusting the daytime dose. This did help but eventually I switched to a pump which gave me far more flexibility, as the basal rate can be adjusted hour by hour. It also gives me a variety of ways of delivering my Bolus doses.
 
Hey & Welcome to the forum. I haven't used touejo, but this is according to nice guidelines in the UK.

"people who are changing their basal insulin regimen from once‑daily Toujeo to a once‑daily regimen with insulin glargine 100 units/ml should reduce their dose by 20%.

Switching from Toujeo to insulin glargine 100 units/ml results in an increased risk of hypoglycaemic events, mainly in the first week after the switch – the dose of insulin glargine 100 units/ml should, therefore, be reduced."

Lada for 5 years...A1c in the 5 percents.Pretty good control.
I have for some inexplicable reason developed a hypersensitivity to insulin,the past 3 months.Originally going low many times at work(physical) on just 3 units of Lantas taken in the morning.Im on 2 Humalog brekkie,1.5 lunch and dinner.
Endo said try Toujeo.For me it's very inconsistent...highs, lows etc.Maybe because of the silly amount I take..2 units.In my mind..the basal I need at night is greater than my daily needs.As you have mentioned...spliting dose... is the nearest chance I believe of getting back on track.Have not done it before..but on paper as it were, seems plausible.Although a pump probably even better..but can't afford one unfortunately.Sorry post is a bit long..but not enough detail would cause confusion I believe.Your post really helped..Thankyou!!!P.s Endocrinology in Mallorca...hmmm not impressed.

Quick question if I may....Why not just split your Lantas doses?Or is Levemir more conjusive for that....a better basal insulin?
 
The profile of Lantus does not make it worth splitting as it’s action time is so long.
Levemir is better for this. It certainly reduced my overnight hypos, but I was still not able to get
satisfactory overnight management, which I was very happy to moved to a pump. I was then able to adjust my background insulin hour by hour, and take account of the dip I have between 3:00 and 4:00.

Around here, anyone going onto a pump is required to make this switch to Levemir with their MDI beforehand.
 
Why don't you start, in order to try and see properly how much background insulin you need and when your body needs it, by doing a Basal Insulin test? It's a bit long winded but diabetes is a marathon, not a sprint - so just more of the same!

Levemir was always intended to be delivered in 2 jabs a day in the first place - but you're on such a tiny dose of it (basal insulin) I'm not sure it would help to be perfectly honest with you.
