I was diagnosed with type 1 diabetes 4 years ago. I find this community very helpful so finally thought I'd join in.

Welcome. I have found it a very helpful and supportive place too.

Glad you have finally de-lurked @Loakie

Tell us a little more about how you manage your diabetes... pens or pump? any fancy gadgets like libre or a smart meter? anything in particular you struggle with?

@everydayupsanddowns I use Novorapid and Lantus pens and a Libre. My main struggle is with Lantus. I happened to stumble across your blog a few weeks ago and found your Lantus posts useful. I've recently moved my basal from 10pm to around 9am to try and reduce the night glucose level drops. It also doesn't last 24h.

Have you actually done a 24hr basal test? - having said that you can't do it in one stint, you divide it into bite sized chunks. See Basal insulins « Diabetes Support Information Exchange (diabetes-support.org.uk)

If you do that and can then see clearly how much or little Lantus is doing for you and when - you can discuss it properly with whoever you see for your D, and most likely get either the insulin changed and/or advice on how to achieve whatever your insulin needs to achieve.

I have considered basal testing but am unsure if it is a good idea as I'm exclusively breastfeeding and don't want to affect my milk supply. I haven't found much information about it. It would be useful to see what the Lantus is up to.

Well in view of the breastfeeding you can't do anything too drastic I agree - but it is only done in 'blocks' which don't have to be all exactly the same length and can overlap, just because the previous one finished at X oclock yesterday doesn't mean tomorrow's has to even start at X oclock. The beginning of another block could start from the middle of a previous one, and finish a couple of hours later. You could do it over 24 days and just do one hour a day - though 12 x and 2 hours would be more manageable.

Who advises you re your diabetes - DSN or GP or who?

I did actually manage an overnight test a few days ago and was stable but then I think my hormones changed again and my insulin requirements increased. That's a good idea about splitting the test into even smaller blocks.

I have a hospital team of DSNs and consultant. It will likely be a while until I get an appointment though.

Can't you get advice via phone or email?

I can phone. Last time I did I was advised to eat 30-40g of carbs uncovered at bedtime. This would have made me hyper, so I had a smaller snack.
