I have had diabetes since 2002 after suffering a large blood clot had to go on insulin two years latter due to stress. Then 4 years ago I came off insulin and put on Metformin but I then could not take the tablets a few months back due to side effects.my glucose levels went out of control and I am under the hospital as the insulin am on is not working .so next week I have appointment at the hospital for them to change the insulin.am worried about the new one working .my diet is good .but am disable with arthritis in the spine and I can’t walk very far. Does any one else have trouble if they have Been taken off Metformin.

That does sound quite difficult but it sounds as if the insulin regime you have may not be suitable. If you are able to say hat insulins you are taking when and what dose then people may have some ideas as to where the problem lies or at least give you some suggestions as to what questions to ask at your appointment.
Just a thought but do you take steroids as they can affect blood glucose levels.

Welcome. What are the names of the old and new insulins, including any letters or numbers there might be after the name. People here can explain the differences between your old and new ones.

No am not on steroids. The insulin am on is Abasaglar16 teen ml one a day in the evening .I also take Gliclazide.I feel like I have been on a roundabout.one of our nurses told me that the people living in our valley with diabetes is the most neglected.the trouble is our doctors cover 3 surgeries.I have spoken to nine different doctors in one and half months and some were not diabetic doctor all had ideas talking to me but not doing much this was dispite that my levels were up to 20,i have had 4 emergency blood tests and one emergency scan.but then had emergency appointment in the hospital clinic were they put me on the insuli.first two weeks were great nurse ringing me To see how I got on .I told her I was concerned that my tea time levels were still high she said she would speak to the dr about putting me on insulin twice a day And would make appointment for me.but then I was just left hanging.had another blood test ordered by my diabetic dr it’s was for the year results not good. Still no one to talk to. But the lady in our surgery did not like how I sounded on the phone she was concerned so she put out emergency call to all our three diabetes doctors.but as Luck had it a nurse from the hospital called and said she had made me appointment for next Tuesday.so am hoping I get better help

The insulin you have been given is a basal insulin which is supposed to keep your blood glucose steady in the absence of food but does not really cope with your meals so that may be why you are experiencing such high levels.
You say your diet is good but what meals do you have?
They may be going to add a bolus insulin which you would take when you eat and that would help with the carbs in your meals.
@Lucyr, @Inka or some others may be able to add some thoughts.

Hi @Ida Shirley Sorry to hear about your difficulties. I hope you can get things improved. A blood sugar of 20 will make you feel rough at the best of times, and when you have other things going on, it just makes them harder.

There are various insulins they could put you on, but your mention of a twice a day insulin made me think that they might be considering a mixed insulin. Those contain both fast and slow-acting insulin combined, and can sometimes help with high sugars, particularly after meals. I see that you’re also on Gliclazide though, so that might mean they have other ideas to try. Another possibility is a twice daily basal/slow insulin. This would be similar to Abasaglar but you’d take it morning and evening, giving good 24 hr coverage and allowing you to adjust each dose separately as needed.

The main thing is that you now have an appointment so will hopefully get some support. Do keep us updated as to what changes they make and how you get on.

My diabetic nurse went through my food cupboard my diabetic Dr been through my diet with me and found nothing with my meals I eat a lot of salads as am unable to cook due to my disability.I don’t eat any sweets .I either eat sugar free meals or things that are low in sugar .I have to eat dairy free foods ,have to have lactose free milk.due some health issues.my highs have only come on since having to come off Metformin.before then I had my diabetes under control and could even have a little treat from time to time.my doctor has told me to stop keep changing my diet As it’s not very helpfu.

I wonder if you are confusing sugar free or low sugar with foods and meals which are low carbohydrate or at least not high carbohydrate and you are actually having more carbs than your medication is allowing your body to tolerate. It is all carbohydrates which convert to glucose.

Hope you can get access to an insulin regime that suits you better @Ida Shirley

Long-acting only insulin can work for some people initially, but depending on how your own insulin-production is working, it can struggle with meals.

2x a day mixed insulins can be a little inflexible, and don’t make it easy to eat meals at different times to normal, skip meals, or have larger or smaller meals than the insulin requires.

You might find it more flexible to add a mealtime insulin which you can take to balance the carbohydrate content of your different meals, and let the background insulin just keep things ticking along in between?

Am hoping to get answers Tuesday as I have a hospital appointment at the diabetic clinic.and the doctor is going over my latest blood tests and then talk me through things .also the change of my insulin.thank you for your advice
