Hi all. just starting on all this message board stuff. I have a 14 year old daughter diagnosed type 1 , 18 months ago. She has always handled all the practical stuff very well. luckily no probs with doing injections, tests etc and has had good enough HbA1c results. She has just started on a pump and we are having a little trouble getting the basal levels right. After a couple of nights of hypos Doc recommended reducing basal rate and now she is high at night. Thinking of returning basal rate to first guess but changing the carb ratio for evening meal. Anyone else tried this?
 
Hi Caro, welcome to the forum 🙂 I don't know a great deal about pumps, but I understand that it is possible to have a number of basal rates set for different times of the day and night, or temporary levels set. Hopefully, some of our pump afficionados will be along to help! 🙂

p.s. you might want to change your profile to 'Parent' instead of 'Type 1' so that people know it is your daughter that has diabetes.

Hi Caro,

I would do this in chunks of 3 to 4 hours.

Start at midnight and set the basal until 3am for a couple of nights and check what levels are at 3am. If they are too high then very slightly increase basal by the smallest increment until you get a reading at 3am that you are happy with, and if they are too low then decrease the basal until your happy with the levels. Then do the next chuck from 3am until 6am and so on and so on.

Remember that each chunk of basal will have a knock-on effect to the next chunk of time, so you cant just decide to do the 6am to 9am basals - unless you have started at 12 midnight and done them in sequence. This is because the midnight levels are 'normally' a good basis to start as sleeping and no real food to deal with give a very steady reading.

Make sure you dont have foods like pizza before doing the first chunk of basal or the results will be out. Perhaps have a very low carb meal like eggs and bacon or something like a salad without bread etc.. this way you are getting a 'truer' reading than having to also deal with meal spikes.
