NHS Diabetes are conducting a survey to find out about peoples experiences of foot care. This information is an important factor in making decisions about how foot care is delivered in the future.

We are seeking information from patients about their experiences in different places and support services they may have accessed to care for an ulcerated foot in the last 12 months.
