Has anyone here had to fill in a Gold Score sheet?

Hey hey, so today i had an email from a nurse, I think as part of this pump approval I've had to fill in a Gold Score sheet with questions about hypos/awareness, alongside a form asking about the impact diabetes has on my mental health, social situations etc..

Did you have to do this? I feel it'd be way easier for them to call me as some of these questions have typos and just don't seem productive at all, plus most of them don't make a whole lot of sense.
 
No, I didn’t have to do anything like that, but I got my pump years ago so perhaps it’s a new thing.

You should point them out to the clinic, they ought to be easy to fix.

Both the Clarke and Gold scores are well respected and thoroughly validated ways of assessing hypo awareness.

I was at a conference presentation some years back by a world renowned expert in hypoglycaemia who asked an audience of consultants and DSN’s with a rather shocked expression,

”Aren’t you checking these annually?! Because you really should be!”

Yeah I'll let them know when I'm next in contact. It wasn't like major typos but a few words that just set me off lol - I bet it was quite cool to be part of something like that, and as the expert had said, they really should be checking!

Hi @olliej96 - my DSN didn’t get me to fill out any forms like the one you mention, but we did go through a sort of handbook covering everything from hypos to carb counting to bolusing for exercise, etc etc. I guess all diabetes departments are different.

I was asked to complete a Hypoglycaemia 2 page questionnaire by my Consultant after I'd asked about moving to a pump. I have very good hypo awareness, which allows me to honestly keep the DVLA at bay; but does make it a bit more difficult for my Consultant to justify the pumping route!

As it happens, I've had other medical challenges (in medical parlance comorbidities) and decided to wait until those are resolved or stable before again considering pumping.
 
Can't recall what is was called but had to do questionnaire prior to getting pump, it covered hypo's quality of life & all that, just answered questions honestly.

I'm with you on the hypo awareness, on the hole I feel more hypos than those that I don't feel, which will mean I'll have to make a stronger case, I guess? Luckily I've not had an issue with the DVLA yet, and would also like to keep it this way.

Im sorry youre facing other medical challenges, I hope you manage to get those things sorted soon!

Yep, seems similar to what I've filled out too! Some of those questions though I do find hard to give a straight answer unless actually talking to someone instead so I can be a little more expressive, but I can understand its easier filling things out.

Part of criteria for being eligible for pump is how diabetes impacts on your quality of life, so if your having more hypers hypos despite basal adjustments or injecting over & above normal then that needs to be mentioned on form.

Had a call yesterday from the nurse after completing these forms, she asked some general questions and asked how it affects me daily, also explained how pumps work (luckily have done research of my own so was on the same page) - they're a board meeting tomorrow in which I'll be discussed. She thinks I'll be a good candidate so let's see..

See I do have hypo awareness in the day, same with hypers I feel them first. But hypos at night we didn't know until I got the libre 1 that I do not wake up from and they happened a lot, but I would be level at waking.
Now with the libre2 and alarms I wake and it is life changing (saving) for me. I would be low for hours at a time with no idea.
