Bosses at NHS England in Leicestershire have told GPs they should keep surgeries open as normal on Christmas Eve and New Year's Eve.
However, GPs argue they should be able to close early as patients do not want to come in for routine appointments.
A spokesman for NHS England's regional office in Midlands and East said: "Primary care services are required to meet their contractual obligations. This includes opening normal hours on Christmas Eve and New Year's Eve, which are working days."
Doctors in neighbouring Nottinghamshire and Derbyshire have reached an agreement to close at 4pm.
