Hi

My medical exemption certificate (for free prescriptions) expires at the end of this month. I was sent a renewal form but it went to my old address, it's been forwarded but has now gone AWOL! i can't seem to find a contact phone number for then anywhere (furiously googling at the moment), does anyone happen to have any paperwork which gives a phone number to contact??

doesnt it say the number on the back of the card???

Wow, that's an amazingly quick reply, thanks everyone! i used the number sugarbum gave and now all sorted.

Admin, I'm very rarely asked for mine when collecting prescriptions which i always find surprising, but I'm just paranoid if I don't have one!
 
has anyone ever NEEDED the certificate? been told the chemist HAVE to see it? ive just said I left it at home and they dont seem to care :D

Nope, never been asked, even when I didn't have one at the beginning - I guess because I'm usually getting insulin or related stuff like needles.

once but i had no diabetes related stuff on script and was using a different chemist for the first time soooooo my own fault really.
