Forxiga.....another pill

Relationship to Diabetes
Type 2
I am a 55 year old male 5ft10 weighing 14 stone and reasonably healthy overall despite type 2. I am now taking four metformin tablets, 3 gliclazide, maximum dose of Victoza injection and now 10 mg Forxiga..........and can only maintain good blood sugar levels by eating little and low carb despite all these medications......does this seem a lot of drugs to anyone ? Also since starting Forxiga had a weird sweet smell to breath and taste in mouth ( for a week ). Anyone else on Forxiga experiemce this ? Thank you
 
Hello @Guardsman ,welcome to the forum. TBH that does sound like a lot if meds to me.
Would you mind answering a couple of questions, they will help us give you the best advise.

How did you come to be diagnosed.
Have you lost weight unexpectedly.
What was your last Hb1ac
Is the smell similar to pear drops.
Are you under a hospitals diabetes clinic , if not I suggest you ask for a referral as they are the real experts, it’s just that I am wondering if you might have been diagnosed as T2 merely because of your age , weight etc, when you may not be .

Was disgnosed about 4 years ago as had classic tiredness and a raging thirst.
Used to weigh 20 st but gradually lost weight which seemed to stall at 16 st but with the help of the Victoza now down to a little over 14 stone.
My mmol/L for the last 7 days has been average 10.8 and average for last 30 days 10.9 but only been on the Forxiga for 6 days so might be too soon to see if thats making any difference.
Going on the Victoza was hard and still causes a lot of nausea and vomiting a couple of times a week but now with the Forxiga I feel pretty rotten most of the time and wondering if the side effects are worth it. I think I will ask for a refersl to the hospital as a second oppinion can't hurt
The taste is similar to pear drops ( lucky I used to like pear drops ) I was concerned about KAD when I first noticed it but its been there for a week so can't be KAD.

It is still possible you have been missed diagnosed Type 2. Slow onset Type 1 or what some call Type 1.5. , often behaves similar to what you have described.
 
DKA can last a week or longer Guardsman, just cos it ain't yet got so bad it's sent you into a coma does NOT mean you don't have any ketones. Usually but not always make me feel nauseous but again sometimes not like I'm actually going to throw up, more like I must have eaten something which evidently my innards don't like, ie vague so I can still function OK as long as I don't dwell on feeling a bit icky.

I strongly suggest, if you do nowt else, nip into a decent pharmacy and ask for some Ketostix cost c. £5. They are urine testing 'dip' sticks.

Your BG levels don’t seem extremely high, but I‘m slightly concerned about your rapid progression through meds and particularly the sweet breath - ‘pear drop’ or acetone breath would be worrying.

DKA is not a risk to be taken lightly, and there is a possibility that you have been losing beta cells over the last 4 years as part of LADA or late onset T1, rather than T2 which might be why you have progressed through the meds so fast.

It would be worth making a phone appointment with your surgery nurse to describe the pear drop breath as soon as possible.
