Driving Licence renewal forms

My forms have just arrived.

The consultant I was under retired during covid, he rang me and told me I would be seen by his replacement.
I’ve never been seen….. I have never seen my GP for my diabetes, ever as I am supposed to be under the care of the hospital.
I have had phone consultations with a DSN, Last one was a year ago. My surgery carry out my yearly bloods, which then the DSN can discuss with me, but have nothing else to do with my diabetes.

My forms have come through to renew my licence. I have no idea what to put.
I never see my GP for diabetes.

I have no idea who my consultant is as I have never seen one since my previous one retired during covid.
What will I put on my form?

Bung your retired consultants name on the form - they really aren't interested in the name, just that you are under the care of someone suitable at an identifiable and suitable place.

Put either, if you’re having annual bloods at the GP Surgery id put them, or else put the previous consultants name.

My reminder arrived this morning and off I went to the website, having fished out my passport, to apply for renewal. All AOK until we got to entering my hospitals phone number - Invalid phone number. Tried everything I could think of, with one space, with 2 spaces, with no spaces. All invalid. Had my lunch. Started again and the form had remembered what I'd put in the boxes so either filled em in for me, or asked if I wanted what I'd told it this morning. We got to the offending phone number and it showed me what I'd typed which started o2476 rather than 02476.

User error ....... prime example.

Two options ring clinic to find out name of new consultant then put their name on form, or put down name of your DSN & make sure to include their full title, that is diabetes specialist nurse next to their name.

It doesn't matter if your last review was by phone, phone calls were common place during covid times & so many people renewed licences during that period.

If you put your old consultant’s name, my guess is they will pass it to the most appropriate person?

More to the point, Covid was a LONG time ago! You really should chase your hospital clinic for an annual review. It sounds like you’ve missed several?
