A medical tribunal has heard how a doctor prescribed "enough insulin for an elephant" to an elderly woman at an Edinburgh hospital.

Dr Lucy Mahinda admitted writing an entry for a "lethal" dose of insulin, which was nearly 10 times the correct amount, at Western General Hospital.

The prescription error was spotted by a nurse before the insulin was given.

Dr Mahinda has been found guilty of making a number of clinical mistakes while working at the hospital in 2012.

😱

It is a weird error to prescribe 38 units. Good that the nurse spotted the error and didn't administer the incorrect, dangerous dose. The more dramatic error of 8 ml instead of 8 units, for example, is actually easier to spot.

Let's hope she's struck off - doctors like her we can do without!
