They all have slightly different qualities ..
Lantus is said to deal with DP very effectively
Levemir is said to be associated with less weight gain
Humalog is American and said to be the nearest thing to the body's own insulin
Novorapid is a product from European Union product, its prevalence in the UK might be down to that or a sweetheart deal between NHS/PCTs ?

Hopefully your med team consider which combination of these is best for you or has particularly good experience of using them.

Ironically none of the four products you mention are actually insulin - they are insulin analogues - manmade concoctions genetically engineered to have certain qualities and to play particular roles as a subsititute for insulin.

thanks for reply whats dp?

My consultant was asked this question when he was taking the DAFNE course I was on..

His answer

Personal preference based on his experience prescribing insulin over the years..

Dawn Phenomenon - Liver puts out a dollopp of glucose at or before dawn to get you ready for the day ahead. Excellent if you are a Caveman, bit perplexing for diabetics who can go to bed on good bgs and wake to an unexplained higher number. Baffles most new T2s.

fair point but i carnt see how you can experience on one day of meeting some one?

In practice, there's so little difference (esp. between humalog and novorapid) that most HCPs will just go with either what they prefer or what they have available.

Lantus has a slightly flatter profile than levemir which may make it last a bit longer. Levemir is meant to be more responsive to changes in dose.

You can't

Hence where the experience comes in from how your other patients have responded to insulin you prescribed it to them..

His example was Levimer and Lantus.. He prefers to start new patients off on Levimer because he has found even though Lantus is supposed to have the flatter profile, he has seen better results with Levimer..

You monitor a new patient to see how they are doing, if they don't seem to be doing well then you swap them to a different insulin to see if that improves matters..

I started out on Animal insulin even though back in '89 human insulin was available, but because I was pregnant my (different consultant) put me on Animal insulin then after a couple of months they change the quick to a different animal insulin to improve control aspects.. The reason for being on the Animal not enough evidence base within pregnant women,

A flat profile is all very well, but no use if your basal requirement is not flat (and I don't know ANY pumpers who stuck to a completely flat profile)

Levemir would have been my next move if I had not moved to a pump, at least it would have given the option of setting 2 different basal levels to match requirement (via 2 injections)

That's the beauty of the pump Mike! :D

I've mentioned this before, that 2 injections per 24 hours of Humalin I, being a medium acting insulin (lasts shorter time than Levemir or Lantus) works well for me, someone who needs very responsive basal insulin regime to match my irregular work and activity patterns - and it's half the price of either of the more used analgues, Lantus or Levemir! Plus, being cloudy, it looks different from clear short acting insulins, so that never in 16 years so far, have I mixed up my insulins.
