Diabetic Review, waste of time for me :(

I was hoping for a fresh start with a newly appointed DN, but felt let down.

1st check blood pressure up to 85 / 146 this was done 3 times to triple check. No kidding its up docs took me off the Ramipril. To which docs have ignored since taking me off them, still have the persistent cough again ignored.

The erectile dysfunction question was asked and all they were interested in was a yes or no answer and no other discussion allowed or listened to.

HbA1c = 45 (corrected from the 48 I was given earlier unless I miss heard).

Cholesterol = 3.3 lowest I have ever been, managed to gets the statins down to 20 mg.

Then went through the repeat prescriptions, which some of the duplicates were removed, same meds at different doses. :(

Weight not taken even though I said I was gaining weight.

Mentioned I was having problems when eating the same meals on different days, that one day I could go high then the next would be low. It was suggested that I split the glic into 2 doses instead of one. When I last tried this went hypo late at night.

Then it was suggested I come off the glic altogether, which would put me back to where I was last Xmas. :(

They did recommend getting the carb and cals book, 🙂

Then went through what I was eating and said too many carbs at once and to split breakfast into two, have half at breakfast and half mid morning. Then add things like eggs to which I already eat 10 to 15 a week. Breakfast is 3 slices of wholemeal bread. Mentioned the dreaded crisps I eat as always hungry (I know crisps are bad).

Then came lunch where it can be bacon, eggs and a couple of slices of wholemeal bread (have to be careful when eating this as BS can and does go low. Alternatives I mentioned like chicken, turkey and ham were ok. Never got to tea and if I had any thing later. Bacon is bad for one to eat according to them and should be off the menu.

The list off what I can and can't eat came out.

Can't eat, sugar, ice cream, biscuits, fruit juice, sweats etc. I played the not sure card here as was getting brassed off by now and not sure what they were after.

Can eat, Bread all types (no way for me white is deadly), rice = yuk, pasta = yuk, then to add insult cereals was on the list = sends my BS sky high. Potatoes were not mentioned but at the moment I rarely eat them.

Then the clincher I am to monitor my levels so they can see what's going on. As it happened I took a print out of my last months levels, and they were not interested :(.

My whole blood count was down, second time I have been told this in a week and it needs attention, doc was not interested when I made the appointment to get it checked out. I am already taking iron tablets 3 times a day.

Some of the discussion was taking place in the doorway as they were running late on the appointments and wanted me out of the way as quick as possible.

Feel really let down as it was as they were preaching what they want to push and not what was suitable for the individual.

Exercise did come into it, fair enough. I explained the ill health and other problems then got a swift lecture about getting it.

Forgot to take my sample with me, so will have to drop one in next week.

I found with the previous nurse that did the reviews (though not a specialist DN) listened acted and gave advise were she could and sorted out more things in the time than got achieved today.

pav, you have every right to rant, sounds most unsatisfactory :( Nothing worse than an HCP who doesn't listen and has no communication skills, just a long list and not a lot of time :( Next to useless. A review where they don't check your weight? Cutting corners to the extreme - takes two seconds. When I was diagnosed I did an OU 'Diabetes Care' course and the main emphasis was communication with the patient being key to a successful review perhaps you should suggest the course to the nurse!
 
Perhaps it would be a good idea for them to do the course :D

Fair enough I nowhere near perfect in what I eat, and I do struggle with foods and get it wrong on occasions. Some times thinking its ok but forget what extras are added in the food.

The weight thing is a major issue with me, as only have to loose about 14 kilos, but the now constant hungry feeling is a right pain. Unfortunately the vegie and green food stuff is also a problem as I can't stand most of it. If I lost the weight it would help other illnesses out as well.

It is so irritating when you don't get time for what you want!

When I went earlier this week got a registrar who wanted to fill in the form correctly. So went over smoking again and when I said I gave up over 20 years ago asked me how many I'd been smoking then!

I was later talking about hypos and she suggested checking the meter in case it's faulty!
Had no idea about using a pump.

She had to leave twice to get some advice...

Resolved me to stick to DSN's - they are generally very experienced.
I know Doctors have got to learn - but I do wish it wasn't with me.

Forgot about the drinking and smoking, she never even asked those questions. I was told she was a DSN :(, which makes me wonder 😱

Think its time to re-look at other GP practices again as now have no trust in the current doctors or the specialist DN, the normal nurses are ok, but don't see them that often.

The reputation of the others I know about that are taking on are equally as bad. 😡
 
Oh dear Pav, no wonder you feel fed up, that wasn't a very supportive consultation at all. As your blood pressure is on the high side, have they prescribed you something else in place of the ramipril?

One point though - I suspect the recommendation to cut out bacon may be due to its high salt content. It's really important to limit your salt intake if you have diabetes and a high BP, to help protect your kidneys. Stick to unprocessed unsalted meats like chicken and turkey.

Nothing has been prescribed in place of the Ramipril, but do take an additional blood pressure med Amlodipine 5 mg as the Ramipril was not working on its own.

BP was originally in the area of 124 / 150 which shows the secondary med is at least helping in some way. I did mention about the blood pressure rising when they took me off Ramipril and they said we should prescribe an alternative and that's as far as it got. :( (to many different docs who can't be bothered to read notes or listen).

Never thought of the salt content in the bacon, recently been buying fresh turkey joints to have for sarnies instead of processed meats, as tastes much better than the pre packaged stuff. I rarely add salt to my food as I feel its burns the inside of my mouth (normal run of the mill salt), on the rare occasion I do add any I use sea salt. I bought a 90g pot several years ago and you can't see that I have used any.

And Errrr - surely if you are anaemic, which you clearly are - any A1c test will be screwed, shouldn't anaemics have a Fructosamine test instead, to get the true picture ?
 
Had to look what the Fructosamine test was and it does make more sense. I never been offered or advised that I should have that test rather than the HbA1c test.

Not surprising in one way I have already had to remind them that they have been issuing meds above the prescribed guidelines (another vote of thanks to the info this forum provides). I have also had to go back to them because the docs prescribe meds that would cause allergic reactions, because they can't be bothered to read notes.
 
How long have you been anaemic? It takes a few months for levels to recover, but if you've been taking the iron for a while and it hasn't had an effect, it needs further investigation. Time to pester on several fronts, methinks.

Been ferreting around found an old repeat script request, would guess its around 2 years when I started taking iron tablets. Was told at the time if levels don't rise I would have to have cameras shoved in various places 😱 😱 another sad tale of once said then to be ignored. I have chased it a few times but falls on deaf ears. 😡

It took many years to get the docs to send me for an X-ray on my knee, by then the arthritis had well and truly set in. :(

Well I think that you really really ought to change your GP before they kill you.

Not being a drama queen, at all.

So you try the one down the road that people also says ain't very good, and see if they are any better. Then you go further afield. If you do a search for a GP on NHS Choices and bung your postcode in, you get shedloads of choices.
 
Ran into a ex work friend who's at a different GP that's had mixed reviews and said their diabetes care has been excellent. Thinking will give them a go as nothing can be worse than mine at the moment.
