Hi,

My name is Matt and I work in the NHS Community Diabetes Team in Salford. We run free education sessions for people living with Type 2 at all the local gateways delivered by healthcare professionals, suitable for the newly diagnosed as well as those looking for further information or support. All you need is a Type 2 diagnosis and a Salford GP (this includes Walkden, Pendleton, Eccles).

The courses we run:
Diabetes Basics - A one-off session to help you better understand and manage your diabetes.

What can I eat? - A single session that focusses on all things food. This is the place to ask any food-related questions.

Living with Diabetes - Three sessions spread over 3 weeks that offer more of a conversational environment to share your experiences.

X-PERT Diabetes - An in-depth exploration of all aspects of diabetes care. This is nationally recognised and runs over 6 weeks with a dietitian and diabetes specialist nurse.

Your GP can refer you in or you can self-refer! Contact the team on 0161 206 5822 to leave a message and we will get back to you. Alternatively, you can email diabetes.admin1@nca.nhs.uk to find out more or book your place.

Hello Matt,
I'm Type 3c (as if T1) and I live 150 miles away from Salford - so not even slightly eligible. But I just wanted to let you know that I think your team sounds like a great initiative. Here on this forum we read numerous pleas from people recently diagnosed T2 then essentially abandoned to find out and fend for themselves what their diagnosis means and what they could do.

Could I ask:
How long has your team been in place?
Presumably its part of one of the recent Integrated Care Structures (ICS) and if so what is your ICS called.
How many members of staff in your team?
Apart from reaching out to this forum how else do you promote your services across the Salford Community?

Keep up the great work.

Hi,

Thanks for getting in touch and the kind words, that's exactly the issue we're trying to fix here in Salford. We find so many who haven't been good advice from the get go!

I'm relatively new to the team myself but I believe they've been doing sessions like this for over 5 years with a team of around 20 people in our community department. You're right to assume we are also part of the Integrated Care Structure within the Northern Care Alliance. They formed in the last couple years merging Salford Royal and Pennine Acute Trusts.

As far as promoting our services go it's a relatively new initiative for us which is part of what I've been brought in to help with. Currently, we set up stands at local events, link with other community services as well as posting on social media @ Salford Diabetes Team all to increase awareness of what we do!
