A project aimed at improving diabetic care across Berkshire West Clinical Commissioning Groups has been shortlisted for an award.
Diabetes Sans Frontières (DSF) formed in 2012 with the aim of reducing the burden of diabetes on people within the community, to improve outcomes, and to reduce the cost of care.
It also aims to reduce referrals to secondary care and the use of emergency unplanned care.
