Diabetes patients set to get 'personal prevention plans'

People with diabetes who do not have their condition under control will be given personal action plans from their GP to improve their health, under a new initiative.

Patients who fail to meet their targets for blood pressure, cholesterol or blood glucose will be given an “information prescription” by their GP.

A one-page document tailored to each patient will contain information the patient needs on how to better manage their health.

An action plan for improving their health will then be agreed with their doctor.
