I am able to put GP annual medication review as the 'last seen' but that might not work if yours is conducted by a pharmacist attached to the surgery, as many seem to be these days.Id just put the date when you did see your GP, and then put all the nurse/DSN appointments in the 'clinics' column,What do you all put in "last seen by GP" I haven't seen mine in a while, mainly practice nurse and DSN at the hospital?
No idea, really, but if they’ve been regular, it all goes to help the picture that you are managing your diabetes, I suppose.I,ve put the DSN details in the consultant section, as I haven't seen a consultant in a couple of years. Do I need to add the clinic dates then as well?

When do you fill that form out? My licence runs to December 2020 - a little way off yet I know and it’ll be the 1st time I need to renew.
Do DVLA contact me or do I need to contact them? Is it a few months in advance?
