This refers maninly to sections 1.4, 1.5 and 1.6

When it talks about Hypoglycaemia and how many events we have had when awake. is this everytime we have had a low sugar or a severe hypo?
 
what does the question say exactly?
 
just tick more than 2 hypos as I bet everyone has more than 2 hypos a year.
obviously you have had hypos whilst awake otherwise you wouldn't be able to answer the more than 2 hypos.
If you don't have hypo warnings then you can kiss good bye to your licence.

Question 1.7 really answers the query for you as it's asking about assisted hypos.
Edited to add the 1.8 question is asking about the assistance in the last 3 months because if more than 2 assists you lose your licence for 3 months I think it is now. Not 100% sure on the time though.

What do you all put in "last seen by GP" I haven't seen mine in a while, mainly practice nurse and DSN at the hospital?

I am able to put GP annual medication review as the 'last seen' but that might not work if yours is conducted by a pharmacist attached to the surgery, as many seem to be these days.Id just put the date when you did see your GP, and then put all the nurse/DSN appointments in the 'clinics' column,
 
I,ve put the DSN details in the consultant section, as I haven't seen a consultant in a couple of years. Do I need to add the clinic dates then as well?

No idea, really, but if they’ve been regular, it all goes to help the picture that you are managing your diabetes, I suppose.

Hi all

I have a question from reading you question!
When do you fill that form out? My licence runs to December 2020 - a little way off yet I know and it’ll be the 1st time I need to renew.

Do DVLA contact me or do I need to contact them? Is it a few months in advance?

I’ve always received the forms through the post well in advance (like, about three months ahead) so look out for them on your doormat from September onwards.

The forms normally come through well in advance but as I need my C1 D1 I have to renew it every year and can take quite a while to get the renewel to come through but as long as your in the process of renewing the licence you can still drive even if its run out
