Confused with results (pregnancy)

I’m currently 16 weeks pregnant. Back in my booking in appointment (December 30th) I had an HBAC1 test that came back at 70, I was absolutely petrified but also very confused as I had no symptoms of diabetes and had an hba1C 1 year prior that had been 38. As I’m pregnant it was instant panic stations as obviously it would seem like my diabetes was completely out of control and had been missed. I was given a blood glucose monitor straight away and was testing normal range throughout the day (4s and 5s both fasting and pre dinners with the occasional 6.4 after eating). I started metformin 3 weeks ago as my FB’s were slightly elevated at 5.7 in the mornings, but had another HBA1C 2 weeks ago, so around a month after 1st one, and it’s come back as 40. Now, I haven’t changed my diet dramatically at all, I still have an office job and I am genuinely confused. I know I am now on metformin but even prior to me starting this my blood sugar readings were fine. Could it be early pregnancy that has caused this elavation/is the initial type 2 diagnosis correct? I have PCOS and I can’t seem to find any research on this in regards to the impact early pregnancy can have on BS /gestational. I’m very aware this is something I now need to control regardless, for life, but I and my consultant seems genuinely confused.

Congrats on your pregnancy. Hope it is all going well apart from this odd high HbA1c result, which clearly doesn't tie in with your finger prick results and I wonder if there was some mix up with the results at the lab. A single high HbA1c should not generally be used as a diagnosis without either diabetes symptoms to confirm it or a second HbA1c above 47 and obviously your second result is below that, so I suspect that first result may not be correct. That said, I believe you are more likely to be insulin resistant with PCOS and different stages of pregnancy you become more or less insulin resistant, so maybe the change in hormones from your pregnancy has reduced the second HbA1c..... however it still doesn't tie in with the finger prick readings you were getting which are all at normal blood glucose levels which points to that 70 result being incorrect.

That said, I have no personal experience of PCOS or pregnancy with diabetes, so perhaps others who do will be along soon with their take on your situation.
 
Congratulations on your pregnancy @Lucystev 🙂 PCOS puts you at higher risk of Type 2 and can make it harder to control blood sugar during pregnancy, so perhaps it’s that? Pregnancy insulin resistance kicks in in the second half of pregnancy, so you might see your sugars going up again.
 
To answer your question, it depends on the individual, but blood sugars generally go higher very early in pregnancy, the lower, then creep up a little and then go up noticeably as insulin resistance hits. From around 30/32 wks IR is very high so blood sugar is harder to control.

I wouldn’t worry as it sounds like you’re keeping a close eye on things - which is exactly the right thing to do. Watch out for any changes.
 
My understanding of pregnancies for ladies with diabetes, is that the first 3 months is Hypo City. The middle 3 are about the same as how their pre conception diabetes management was, and the final trimester is HYPER City. With much much more frequent testing from start to finish.
