Bolus for slow releasing foods

Hi, I need some help with understanding how some foods release slowly and how to bolus without getting high readings in the night.

My son who is 27 has Down's syndrome and we look after his insulin requirements via the pump. In general we have good control of his blood sugars but have a lot of problems when he eats out, pizza, pasta, chips etc.

Here is a recent example of the problem.
Chips and sausage from the chip shop.
6 units bolus using multi wave with 3 initially and 3 over 5 hours.

Blood 6.3 before eating
Multiwave Bolus given at 636pm
Blood at 8pm 6
Blood at 10 pm 6.9
Blood at 11.43 8.8
Blood at 1235 am 12.1
Blood at 5 am 16.9 bolus 5 units
Blood at 930 am 9.3.
I think I would of given a correction bolus of 1 unit at 1235.

We have to be very careful not to over bolus as we are not always aware if he is hypo so at night we edge on the side of caution.

I know everyone is different but I would like to know how others deal with such foods and how long they use multiwave or TBR's for. I've tried different % on multiwave and tried extending how long the multiwave delivers for but still no joy.

I am not an Insulin user but from what I have read from others it is a difficult one.
I am sure someone will be along to help.
 
Forever a problem when carbs and fat are concerned I'm afraid. Try to make that type of food a bit of a treat rather than a usual thing. It's not him being peculiar incidentally - it's all of us to one extent or another. Even with a pump and multi-wave or extended boluses, I need two if I eat more than approx. 50g carb that's also fatty (eg pizza, or rice or pasta with a fatty sauce) which is a pain cos the second spike doesn't start until approx. 5 hours after starting to eat - so it's bedtime or after bedtime unless I eat about 5.30 or 6, which is rare.

Hence - I limit the amount of goodies I eat which meet this description, cos I don't want to entirely give em up.

I only found out this magic figure of 50g by test test testing after eating whatever meal it was, on a few occasions and specifically noting the results and comparing them till I found the amount I could tolerate.

On the odd occasion I do eat more for whatever reason, I don't worry about it and just sort it out next day. But it's not very often I allow myself to do it.
