Hallo please can you help to simplify and clarify basel tetsing. At the moment my insulin needs are 14 unit of lantus at 7pm and a 1:10 ratio of Novorapid. Now next week i want to commence the basel testing due to falling off the diabetes and excerise wagon therefore i thought i would give my whole diabetes regime a shake up by starting at step one again 🙂 So looking at the various links that have been passed on this subject can you confirm if i start next monday do i start with the nightime testing first or breakfast? Then would i do every other day for the breakfast test so that would mean the wed then friday. (obemsite mentioned 3 days hence)It has also said check bloods every 2 hours thats fine but one site said your starting and ending test should not be either in the 10% up or down or 1.7 up or down - which one is it ? I know you have all been asked this question before but for me i just need a simple equation to get myself back on track please. Thanks for your help - i know i sound thick as mince but sometimes i cannot grasp the obvious

Personally I go for 1.7mmol/L since with meter inaccuracy pretty much any result within a range of 1.7 could be exactly the same.

As to when to start - I'd say whichever order is most convenient, and not necessarily in any kind of chronology. Test each period twice if possible to help rule out 'one of those days' events.

You should be able to choose when to start your basal testing Vanessa, it doesn't have to be at any particular time - whatever suits you best 🙂 Also, the idea is that your start and end tests should be quite similar which will show that your basal is managing to hold your levels steady when you are not eating. So, the 10% or 1.7 are simply a way of confirming this - it depends on what your levels are actually like at the start. If, for example, you were 4.5 to start with then a 10% change would put you at 4.1 or 4.9 (approximately). You wouldn't want to start at 4.5 and drop by 1.7 though because that would leave you at 2.8 and mean your basal was too high!

Thankyou both for your replies much appreciated will start on the breakfast ones first, I already know I get the "feet to floor" or foot to floor whichever one is correct lol - reading it will be interesting to see how long it lasts for..... Also just to confirm I will have to do these tests again when I start properly training again - running and the gym will be the training........Will post results as soon as i start cheers
 
Breakfast Basel Test 1st one!

Hi here are the results of my first breakfast basel test today:

So had 14 units of Glargine at 7pm last night:

7am - 4.7 - on waking
8am - 7.0 - this jump would be feet to floor
9am - 6.8
10am - 6.7
11am - 5.9
12am - 5.5

I wont post on each set individually (as you will all get bored) I will post when they are all done just wanted comments on this set as its the first one I have done... 🙂

Those look pretty steady to me Vanessa 🙂 I think I would be happy to see numbers like those.
 
I agree that your basal dose is doing what it should be, at least between breakfast and lunch......

You had a wee rise up to 7 in the space of an hour, Dawn Phenomenon maybe, although you didn't continue to rise later.....

Why cant we all follow the same rules.........:D
 
Agree they look fairly steady. Might be worth running the same time period again to check. Your fall from 7 to 5.5 after your FHTF/DP has calmed down might suggest *very* slightly too much basal at that time of day - but without a mini-bolus to cover the liver dump after waking you seem to be ending the period pretty close to where you started which is good news.

The other alternative might be to reduce basal slightly and alter your breakfast dose by approx 3mmol/L's worth of 'rise' to combat the liver actvity.

Personally I wouldn't do anything until you've seen what happens over other time slots though as one dose of Lantus is not particularly flexible and these results are pretty solid.
 
Well I vote we wait for the full 24 hours results before suggesting anything, frankly! cos it may be poss to nudge the evening Lantus forwards a bit (to 9pm or later) and get a different result from the Lantus peak being closer to the FHTF.

But can't say try that unless we see what's happening rest of the day.
 
Northener if these readings lead me onto the numbers and HBAC that you achieve then I would be a happy bunny ! Although would prob have to up my running alot lol........

Thanks TW and Everyday yep I will be continuing with the full 24 hr sets and 2 shots at each set so another breakfast to go! I think I will be lucky to see figures like these in the other times but you never know 🙂 Will post when all done and will prob need more helpful suggestions so def keep em coming cheers guys

And Novo if we all followed the same rules i would be bored, 😉 i mean how much fun do we all have when we get those random highs and buffet induced hypo's when miscalculating the insulin for those party food treats - fell into that trap at christmas buts thats for my next stage of bolus calcs to come once ive sorted this basel stuff!! One step at a time I must remember 🙂
 
Well we went for a walk this afternoon and it was meant to be half an hour but when we got to where we intended to turn round we said, oh shall we carry on and go 'right round the block' ? which made it over an hour.

Hence although it was an hour after lunch when we started and I was at 7.0, by the time we came back I was 3.6 and despite consuming a 42g CHO mince pie with my cuppa, I was still hypo until I started cooking dinner 2 hours later!

I hadn't even thought about a TBR or less bolus as it wasn't at all energetic really.

Still the mince pie was a nice treat!

Walking for anything over 15 minutes makes a big difference to my sugar levels. I usually walk after a meal so I can manage it by reducing the bolus, which I assume is what you'd have to do as fine tuning Lantus doses is pretty much impossible!

Have you tried eating a small amount of protein as soon as you wake up to deal with the morning rise?
