I use Myfitnesspal, (the free 'light' version) It has a wide database of foods, and you can log your own recipes into the memory. Other people swear by Carbs and Cals, which has an app and a book form, because it has pictures of foodstuffs, which helps guesstimate carbs in stuff when you're faced with a plate of something when you're out.
