Another doseage question (amazingly fed up)

Hi

I'm aware that the majority of basal rates are set at about 50-60% of your total insulin for the day. Eg: total insulin = 60u then your basal maybe set at around 30-35u and the rest would be your rapid?

Well on an average day I do not take more than 15u of rapid insulin. (1:10g carb ratio)
Yet my basal rate set by the hospital is 27u of insulatard. Why would they set it so high?

I'm still getting horrid high pre-meal readings and morning readings

Any help would be great as I'm getting to the point of throwing in the towel completely :(
 
My basal used to be around 25% of my TDD (Total Daily Dose), then it started declining - now I'm on 0% basal 😱 I think these are really just guidelines - there are various 'rule-of-thumb' calculations that can be done when setting the initial fast/slow ratio, but really it is down to lots of testing and experience that decides what your ratio ought to be.

I actually thought it was supposed to be the other way around - that most people start on 40% slow and 60% fast. Of course, it is also going to be affected by the amount of carbs you consume in a day - your basal, once set correctly should remain relatively constant, but if you don't have a lot of carbs then your fast-acting needs are going to be lower than 'average'.

I would ask them for their reasoning behind your current split. If it's really not working for you then it sounds like they need to apply much more thought to what needs tackling.
 
All my pre meal levels seem to be running between 7-13 in the last week. Even though I've upped my basal on advice from the hospital.

Feel like I have an almost constant headache which ranges from a background to a full blown migraine pain level.

I just want this fixed so that I can enjoy food again instead of dreading it
