I've recently been diagnosed with diabetes and have now realised how my dad does not take care of himself as he has had it for about 5 years now. I really don't know what to do! I tell him to at least check his bg but he avoids it and I've tried to create meal plans but he doesn't listen. What else can i do?

Welcome to the forum, Riri2304.
So much depends on your Dad and your relationship, including how much time you spend together, how often you eat together, how often you are in phone contact, his philosophy of life etc.
Your Dad has to manage his own diabetes, although support / setting an example can be important / valuable too.
