I'm type 1 diabetic taking humalog / QA before breakfast, lunch & tea then taking lantus / BI before bed.

A few weeks ago i was getting very high blood sugars on waking / first thing in a morning so 2 weeks ago i put my BI lantus up from 30u to 32u & now i'm getting lots of hypos so am wondering whether to drop it back to 30u.

Thank you in advance for any help.

I'm a total beginner when it comes to insulin having only been on it a week, but reducing the dose makes sense to me. I'm sure there will be someone along soon who knows more.

It's tricky when you are on a dose above 24 units as single unit lantus pens only go up to 24, and it sounds like you might benefit from being able to adjust by just one unit at a time rather than 2. An alternative might be to get a single unit pen and then split your dose into two injections - I would have a word with your DSN to see what he/she says. Of course, it may be that just lowering the dose to 30 units again will do the trick as there may have been other factors at play that were making your levels go high before.

splitting it will defo be better for you, just keep recording and present the results to your team.......😉

The pen which i put my lantus cartridge is the Autopen 24 which goes up to 40

But there's also a Green version which dials doses in 1u increments so you can go up in 'ones'. Unfortunately as Alan says that one only goes up to 21u.

But it does sound like 2x doses of Lantus approx 12 hours apart might give you more options flexibility - you can start with them evenly split (say 16u and 16u or 16u and 15u) and then more it from there to give you more overnight and less during the day if that's what you seem to need

Thanks for all your replies so far, had a good read of them all.

Think i might reduce / tweak my BI / lantus dose back from 32u to 30u tonight. Do what the rule is ie wait 3 or 4 days so see any changes or as my DSN (on my last appointment) said to me she would be more inclined to wait for 1 week / 10 days. Then if i'm back to high morning blood sugars i will think about splitting the dose.

Re splitting the dose of BI / lantus i don't know whether to just go ahead and do this myself or ask my DSN first as it seems quite a scary thing to do ie if i'm suddenly taking only half my amount of BI at bedtime then won't my bs be more than extremely high the next morning???

Hi I just wanted to say that I hope that changing your doses works well for you. A few years ago I split my levemir doses and that has been really good for me - but I'm afraid that I can't remember how I did it with regard to the first cut down dose of levemir - it probably is best to check with your DSN
