A change to my insulin?

Hi there, so I’m 38 and been a type 1 diabetic for 25 years. Never had any problems but for the last year of so I’ve been having loads of quite severe lows. Just come from my routine appointment and my hba1c is 66 which seems to contradict all the lows I’m experiencing. I’ve been on novorapid and Levimir for 12 years, is it just time for a change of insulin? What works for you guys?

When on MDI I found that Levemir split morning and evening, along with Novorapid gave me the greatest flexibility. I liked being able to change the basal for daytime on a busier day without impacting my night time dose. I did find that I was still having hypos even with this as I was on small doses, and this prompted me to switch to a pump.

Your HbA1c is an average so even if you are having lows these may be balanced by highs so end up with an average that you are happy with. Have you talked to your team about accessing the Libre sensor? This will show your sensor glucose throughout the day, and show the amount of time during which you are in range. This is in many ways a lot more helpful than the HbA1c, and I have found that my more recent appointments with my team focus on this a lot more than HbA1. Well worth discussing with them, and quite an eye opener. It certainly helped me head off both highs and lows, and also showed me which foods were particularly spikey for glucose levels.

You need to go back to basics and do a basal test so you know your basal is correct, then start looking at your bolus/carb ratio.
When are you having the hypos in relation to when you have eaten/bolused for a meal?
 
I don’t think you necessarily need to change insulins. Just check that your current basal amount and meal ratios are correct. It could be you need a few tweaks.
